Active Solutions Inc. is currently seeking a Mi Via Program Coordinator for our Mi Via Program that will be a great fit with our ASI family and who is able to help support individuals and families who receive services through the Mi Via Waiver. Active Solutions Inc. will provide training and mentoring so that we can support you and enable you to do your best!! The Mi Via Program Coordinator will work hand in hand with individuals, Mi Via Consultants, Employer of Records (EORs), Financial Management Agencies (FMAs), Direct Support Staff and other team members.
The Mi Via Program Coordinator will be responsible to ensure that all services being provided meet the requirements of the Mi Via Waiver Standards. The Mi Via Program Coordinator will work with Individuals, Family Members, Mi Via Consultants, Employer of Records (EORs), Financial Management Agencies (FMAs), Direct Support Staff and other team members to ensure that individuals are receiving the best possible services.
Duties Include:
- Conduct meetings with individuals and Direct Support Staff. Purpose of the meetings is to review status of individuals, update QA spreadsheets, discuss any issues they may be experiencing, problem solving, etc.
- Communication MUST be timely, responsive and thorough.
- Point of contact for ALL MiVia services, questions, problems and concerns.
- Encourage team work and positive attitudes.
- Open door policy is required. This means that Program Coordinator must be accessible and approachable.
- Assist Operations Manager in rolling out and implementing policies and procedures.
- Ensure health and safety of the individuals served through monitoring of Direct Support Staff.
- Complete reports, documentation, billing and invoices in a timely manner and ensure that payments are being received for services.
- Ensure that any issues with payments or Conduent are resolved in a timely manner in order to ensure Direct Support Staff are paid timely.
- Fill out ANE Reports on behalf of individual. Ensure individuals are free from abuse, neglect and exploitation. Report if any of these are suspected, per HCA regulations. Ensure Direct Support Staff follow policies and procedures for reporting.
- Ensure Direct Support Staff complete notes within 48hrs of provided services.
- Assist individuals and Direct Support Staff with any issues.
- Update and maintain individual QA/QI spreadsheets to ensure master files are current.
- Notify Operations Manager of any serious problems/issues that Program Coordinator needs assistance or direction with.
- Complete all state and agency trainings as required, per timelines recommended by DDSD/HCA.
- Must have thorough knowledge of Mi Via Waiver Standards and services provided by ASI and remain up to date on any changes in requirements.
- Attend ASI Manager, staff, team and other designated meetings.
- Attend Bi-monthly meetings with ASI President.
- Advocate for individuals and support them with efforts at self-advocacy.
- Ensure master files are kept current. Archive out of date documents. Conduct master file review of individual files quarterly.
- Must be able to work with Individuals, Family Members, Mi Via Consultants, Employer of Records (EORs), Financial Management Agencies (FMAs), Direct Support Staff and other team members to ensure that individuals are receiving the best possible services.
- Be available by phone and text message.
- Respond to phone calls, emails and text messages within 24 hours.
- Model professional behavior and communication when representing Active Solutions Inc.
- Complete all other duties as assigned.

- High school diploma required (Bachelors or Masters Degree preferred)
