Full Job Description

POSITION SUMMARY: Under the supervision of integrated BH Coordinator provide the integrated behavioral health services at AHS Family Health Center (AHSFHC). Provide Substance Use counseling to AHSFHC clients.

DUTIES AND RESPONSIBILITIES:

Assessment of mental health needs, treatment planning, coordination and implementation of behavioral health services for patients.
Has thorough knowledge of current trends in local, state and federal programs in the behavioral healthcare field.
This position is a member of the multidisciplinary, primary care team and communicates and interacts with all levels of staff, primary care providers and health center patients.
Responsible for the adherence and maintenance of Mental Health policies and procedures for the behavioral health program.
Evaluates and assesses (provides intakes) for patients in a timely and courteous manner.
Performs counseling and crisis intervention as necessary.
Refers patients with serious mental health conditions necessitating inpatient or specialty care to an appropriate institution or agency outside of AHSFHC.
Refer patients to MAT services.
Responsible to screen patients for prescription drug abuse, illicit drug abuse and alcohol abuse - Provide counseling to patients who screen positive or refer them to appropriate resources available in the service area.
Responsible for a caseload, providing mental health assessments annually, individual treatment plans and reviews every 6 months; if patients are in need of case management, refer patients to Uptown mental health program.
Process patient records within 24 hours for billing using EMR.
Annual productivity should correspond to the minimum national average of a LCPC/LCSW in a FQHC setting or in accordance with the program expectations of the funding organization. Productivity expectations will be prorated accordingly for full time LCPC.
Serves as field supervisor for student intern in field placement/internship as appropriate.
Contributes to the training and proficiency of clinical personnel as appropriate.
Participates in the development of the AHSFHC ITP healthcare plan.
Participates in the quality improvement program that will assure that patients receive quality care in the office and from other providers of health care services, using peer review, medical record audits, grievances and other mechanisms as appropriate.
Attends administrative team meetings, care coordination meetings and organization-wide staff meetings on a monthly basis.
Maintains professional competence by pursuing opportunities for continuous learning, attending continuing education offerings, reading professional literature, and engaging in other activities, which develop new knowledge and skills.
Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, Mental Health Confidentiality Act and other State and Federal laws).
Maintains License by attending CEU trainings.
Performs other duties as assigned.

QUALIFICATIONS:

LCPC or LCSW with a Masters degree from an accredited school required.
Current license to practice as a LCPC or LCSW in the State of Illinois.
Demonstrates excellent clinical skills
Evidence of mature judgment and crisis intervention skills.
Ability to effectively present information both orally and written, to administration.
Ability to work as a team member.
Computer literacy is required and ability to process patient records electronically.
Ability to work with a culturally diverse team.
Must be willing to work flexible hours as required by position.

Pay: $65,000.00 - $75,000.00 per year
