We currently have an opportunity for a Bi-lingual Chinese-speaking Registered Nurse in Brooklyn to make visits to patients' homes.
Key Functions:

Perform initial assessment of clients for admission to home care, reassessments, and post-hospital discharge visits

Supervise and instruct PCA/HHAs
-
Review medication
-
Develop aides care plan, and MD orders in HHA Exchange
-
Collaborate with medical providers, and other members of the interdisciplinary team

Work closely with coordination department
-
Performs patient/family education
-
Discharge planning
Qualifications:
-
Current NYS RN License & Registration required
-
Bilingual English-Chinese required
-
Home care/experience with adult/geriatric population preferred

Must be willing to travel to clients homes

Ability to use computerized systems
-
Excellent verbal and written communication skills
-
Customer service skills are essential
