CC Young, a vibrant senior living community and a non-profit leader in senior living and Health Care, is seeking a Full-time Licensed Vocational Nurse (LVN) to join our Home Health Team! Under the guidance and supervision of a Registered Nurse (RN), you will provide high-quality skilled nursing care to patients in the comfort of their homes. All care is delivered in accordance with physician orders, agency policies, Texas state regulations, and Medicare Conditions of Participation.
8:30am- 5:00pm Monday - Friday, with periodic on-call during the week and occasional weekends.
Reports To: Clinical Supervisor / Director of Nursing (DON)
Key Responsibilities include, but are not limited to:
Clinical Care
- Provide skilled nursing services as ordered by the physician and outlined in the patient Plan of Care (POC).
- Perform nursing procedures within the LVN scope of practice and agency policy.
- Administer medications and treatments as prescribed.
- Perform wound care and dressing changes.
- Monitor vital signs and assess patient response to treatment.
- Provide catheter care, ostomy care, tracheostomy care, and enteral feeding management as applicable.
- Assist with infusion therapy and central line care as permitted by state regulations and agency policy.
- Recognize and report significant changes in patient condition to the supervising RN and physician.
- Reinforce teaching provided by the RN regarding disease processes, medications, safety, and self-care.
- Performs other duties as assigned.
Documentation
- Complete visit documentation accurately and timely in the electronic medical record (EMR).
- Document skilled need, interventions performed, patient response, and progress toward goals.
- Ensure all documentation meets Medicare, Medicaid, CHAP, and agency requirement.
- Submit required paperwork and visit notes within established agency timeframes.
Care Coordination
- Collaborate with the RN Case Manager, physicians, therapists, social workers, and other healthcare team members.
- Communicate patient status and concerns promptly.
- Participate in case conferences and interdisciplinary meetings as requested.
- Assist in implementing and revising the Plan of Care under RN supervision.
Patient Safety & Compliance
- Follow infection prevention and control standards.
- Adhere to HIPAA privacy and confidentiality regulations.
- Maintain compliance with agency policies, state regulations, CHAP standards, and CMS Conditions of Participation.
- Report incidents, accidents, abuse, neglect, or safety concerns immediately.
- Ensure patient home environment safety and report hazards.
Quality Improvement
- Participate in agency quality improvement activities.
- Assist in achieving agency quality outcomes and patient satisfaction goals.
- Maintain productivity standards established by the agency.
Minimum Job Requirements & Qualifications
- Education: Graduate of an accredited Vocational Nursing program.
- Licensure/Certification: Current, unrestricted LVN license in the State of Texas; current CPR/BLS certification.
- Experience: Minimum of one (1) year of recent clinical nursing experience. Home health and advanced wound care experience are highly preferred.
- Skills: Strong clinical assessment, time management, and communication skills; proficiency with Electronic Medical Records (EMR).
- Travel: Valid Texas Driver’s License, clean driving record, reliable transportation, and current auto liability insurance.
Physical Requirements & Working Conditions
- Ability to stand, walk, bend, kneel, and lift up to 50 pounds.
- Comfortable driving and traveling throughout the designated service area to provide care in various home environments.
- Potential exposure to communicable diseases and varying home environments.
Performance Expectations
- Complete documentation within agency timelines.
- Maintain productivity standards.
- Demonstrate professionalism with patients, families, referral sources, and team members.
