Hospice Registered Nurse (RN) Case Manager - Full Time

Topeka, KS Metro Area

Make a Difference with Faith Home Health & Hospice

At Faith Home Health & Hospice, we are committed to providing compassionate, patient-centered care that brings comfort, dignity, and support to individuals and families during life's most meaningful moments. We are seeking an experienced Full-Time Hospice Registered Nurse (RN) Case Manager to join our team and serve patients throughout the Topeka, Kansas area. Specific territory details will be discussed during the interview process.

As a Hospice RN Case Manager, you will provide skilled nursing care to hospice patients in their homes, develop individualized care plans, and collaborate with an interdisciplinary team to ensure the highest quality of care. This role offers the opportunity to build meaningful relationships while supporting patients and their loved ones through every step of their hospice journey.

Please note: A minimum of one (1) year of hospice experience is required for this position.

Key Responsibilities

As a Hospice Registered Nurse (RN), you will:

Manage a caseload of hospice patients and develop individualized plans of care.
Perform comprehensive patient assessments and ongoing evaluations of patient status.
Update and revise care plans as needed to meet changing patient needs.
Provide education and support to patients, families, and caregivers.
Deliver skilled nursing services, including wound care, symptom management, disease management, and medication review.
Coordinate care with physicians, caregivers, and members of the interdisciplinary team.
Maintain accurate and timely clinical documentation.
Advocate for patient comfort, dignity, and quality of life.

Qualifications

To be successful in this role, you will have:

A current, unrestricted Registered Nurse (RN) license in the State of Kansas.
A minimum of one (1) year of hospice nursing experience required.
Two years of clinical nursing experience preferred.
Current CPR certification.
A valid driver's license and reliable transportation.
Strong assessment, communication, and time-management skills.
Availability to work Monday through Friday and participate in an on-call rotation.
