Oakland Heights Nursing and Rehabilitation and Lake Park Healthcare Center are seeking an experienced Director of Rehabilitation to provide unified therapy leadership across both facilities in Oakland, California. Oakland Heights Nursing and Rehabilitation is a 48-bed skilled nursing community. Lake Park Healthcare Center is a 35-bed skilled nursing community. Compensation and Financial Security
The annual salary range for this position is tiered by licensure as follows:

Physical Therapist or Occupational Therapist: $135,000 to $165,000 annually, commensurate with experience

Physical Therapist Assistant or Certified Occupational Therapist Assistant with five or more years of DOR experience: $100,000 to $125,000 annually, commensurate with experience

The Role
The Director of Rehabilitation provides strategic, clinical, and operational leadership for rehabilitation services at Lodi Nursing and Rehabilitation. This position is responsible for ensuring the delivery of high-quality, compliant, and financially sustainable therapy services across Physical Therapy, Occupational Therapy, and Speech Language Pathology disciplines. This position is classified as Exempt and reports to the Facility Administrator.
Core responsibilities include:

Providing strategic and operational oversight of all therapy services, ensuring clinical excellence, regulatory compliance, and financial sustainability

Recruiting, hiring, onboarding, mentoring, supervising, and evaluating therapy staff across PT, OT, and SLP disciplines

Developing, implementing, and monitoring therapy program standards, protocols, and workflows that reflect current evidence-based practice

Managing scheduling, caseload distribution, and productivity to ensure operational efficiency and quality patient experiences

Monitoring and managing therapy key performance indicators including productivity, outcomes, patient satisfaction, and program performance

Ensuring compliance with all applicable federal, state, and California skilled nursing therapy regulations, payer requirements, licensure standards, and accreditation guidelines

Overseeing accurate and compliant billing, coding, documentation, and MDS participation in collaboration with the interdisciplinary team

Serving as the primary clinical liaison between the therapy department and facility administration, communicating program performance, needs, and opportunities clearly and proactively

Leading therapy staff meetings, performance reviews, and professional development initiatives

Maintaining a personal clinical caseload as appropriate to the program model and operational needs

Maintaining compliance with HIPAA, patient rights, OSHA, infection control, and all applicable California state practice standards

Treating all residents, staff, and community partners with dignity, respect, and compassionate professionalism
Physical requirements include the ability to assist with patient transfers, mobility support, repositioning, prolonged standing and walking, bending, reaching, and lifting with or without assistive devices. The ability to assist in resident evacuation during emergency situations is also required.
What You Bring

Current, active, and unrestricted license in the State of California in Physical Therapy, Occupational Therapy, Physical Therapist Assistant, or Certified Occupational Therapist Assistant — required

Graduate degree in Physical Therapy or Occupational Therapy from an accredited program, or Associate degree conferring the PTA or COTA title — required as applicable to discipline

Five or more years of experience as a Director of Rehabilitation or equivalent therapy department leadership role — required for all licensure types

Prior experience as a Therapy Department Director or equivalent leadership role — strongly preferred

Demonstrated knowledge of MDS, PDPM reimbursement methodology, and skilled rehabilitation regulatory requirements

Strong verbal and written communication skills with the ability to lead by example across clinical and administrative settings

A collaborative, solution-oriented leadership style that builds team trust and accountability

Composure and professionalism in a dynamic, fast-paced clinical environment

Current CPR/BLS certification (or willingness to obtain per facility policy)

Ongoing compliance with continuing education requirements established by the applicable California state licensing board
