Full Job Description

The Hospice Care Consultant is responsible for building relationships with referral sources, being able to communicate the services provided by The Connecticut Hospice. The Hospice Care Consultant will have a close working relationship with other departments such as Admissions, Business Services, and Finance. This position is a "hands on" position which includes travel to other facilities such as physician's office, long-term care facilities, and hospitals. Must support the Connecticut Hospice's vision, mission, and operating principles.

Overview:

The Hospice Care Consultant is responsible for sourcing patient access through direct community contacts, SNF’s, ALF’s which educate healthcare providers and the general public about the Hospice Program. You will be assigned accounts with 50 to 60 contacts and should establish relationships with assigned referral sources targeting 6-10 contacts/meetings per day and utilizes Sales Force to capture all books of business (BOBs)

General

Increase knowledge and education so our community members can participate in hospice and palliative care as their health care needs
Initiating the process to help a patient and family start hospice services
Educating physicians, nursing homes, assisted living facilities, hospitals and other healthcare partners on the life-changing benefits of hospice care
Ensuring patients facing life-limiting illnesses have access to the care and support they need to live each day to its fullest with comfort, dignity, and quality of life.
Demonstrates initiative and commitment to achieve company growth and results to ensure long term financial stability.
Represents the company positively and professionally within the community.
Provides accurate information regarding hospice services in response to inquiries by healthcare providers and general public, in accordance to the goal-directed development.
Maintains current data on market area, competitors, and marketing.
Maintains an organized approach to territory and reports all activities in Sales Force
Prepares and conducts call and presentations to potential referral.
Participates in strategic planning and the analysis for their assigned territory in conjunction with the business.
Coordinates with clinical management staff in planning in-service and presentations, and in addressing issues with referral sources.
Participates in community and organizational programs as requested to promote professional growth and understanding of hospice care.
Conducts business effectively and in a fiscally responsible manner, and reports cost effectiveness of marketing efforts.
Initiates and coordinates contract negotiations with facilities, insurance companies and managed care
Proactively building positive relationships with referring physicians; facilities; case managers discharge planners and social workers in hospitals and skilled nursing facilities; and other health care providers.
Coordinating public education and communication efforts to increase community outreach.
