Job Summary:

We are seeking a highly motivated and compassionate Physical Therapist to join our team at North Lake Care Center, a provider of exceptional sub-acute medical care, rehabilitation services, and 24-hour skilled nursing care. As a Physical Therapist at North Lake Care Center, you will have the opportunity to make a meaningful difference in the lives of our residents and patients while working in a supportive and collaborative environment.

Responsibilities:

Conduct thorough evaluations of patients to identify physical therapy needs and develop personalized treatment plans
Implement and modify physical therapy programs to achieve optimal patient outcomes
Provide direct patient care, including exercises, stretching, and other interventions as needed
Collaborate with other healthcare professionals, including physicians, occupational therapists, and speech-language pathologists, to ensure comprehensive care
Maintain accurate and timely documentation of patient progress and treatment plans
Participate in patient care conferences and communicate with patients, families, and healthcare team members regarding patient status and treatment plans
