Position Summary
The Primary Care / CDU Physician provides comprehensive, patient-centered care across both outpatient and observation settings, with clinical responsibilities equally divided between Medical Home Southwest (outpatient clinic) and the Clinical Decision Unit (CDU).
This hybrid role combines longitudinal primary care with acute episodic care, supporting continuity, access, and efficient patient flow across the health system. The physician collaborates with multidisciplinary teams and may participate in teaching, quality improvement, and academic initiatives within a dynamic public health system.
Essential Duties and Responsibilities
Outpatient Clinical Responsibilities – Medical Home Southwest (50%)
Provide comprehensive primary care to an assigned patient panel, including preventive care, acute visits, and chronic disease management
Coordinate multidisciplinary care delivered to their panel to optimize patient outcomes
Diagnose, treat, and manage acute and chronic conditions using evidence-based guidelines
Deliver preventive services, including screenings, immunizations, and health education
Review and interpret patient medical records to guide clinical decision-making
Make appropriate referrals and collaborate with specialists and ancillary services
Maintain accurate and timely documentation in the electronic medical record (EMR)
Participate in care team huddles, population health initiatives, and panel management activities
Demonstrate patient-centered and patient-valued behaviors to enhance patient experience and satisfaction
Serve as a resource to patients, families, and the community, including participation in outreach activities as appropriate
Collaborate with residents, medical students, advanced practice professionals, and staff in a teaching environment
Participate in performance improvement programs, peer review, and departmental initiatives
Clinical Decision Unit Responsibilities (50%)
Provide direct patient care in the CDU, managing observation-status patients requiring short-term treatment and monitoring
Perform initial evaluations, ongoing assessments, and disposition planning (admit vs. discharge)
Manage acute conditions appropriate for observation care, ensuring efficient and evidence-based treatment
Collaborate with emergency department physicians, specialists, nursing, and case management
Facilitate timely patient progression and disposition to support hospital throughput and access
Ensure accurate and timely documentation in the EMR
Participate in interdisciplinary rounds and care coordination activities
Support hospital initiatives related to patient flow, observation care optimization, and quality metrics
Participate in shift-based coverage, including weekends, holidays, and call schedules as assigned
Additional Responsibilities
Participate in departmental meetings, committees, and faculty development activities where appropriate
Contribute to quality improvement, patient safety, and performance initiatives
Support institutional goals related to patient experience, access, throughput, and clinical outcomes
Partner and communicate effectively to promote a positive experience for patients, staff, and community stakeholders
Maintain professional licensure, credentialing, and continuing medical education requirements
Teaching Teams:
Assistance with rounding on teaching teams as needed as part in inpatient work.
Supervise and teach family medicine residents, medical students, and other learners in the inpatient setting.
Provide bedside teaching, case-based learning, didactic instruction, and mentorship.
Foster a positive learning environment that supports professional development and clinical excellence.
Evaluate learner performance and provide timely, constructive feedback.
Participate in residency program curriculum development and academic conferences.
Support scholarly activity, quality improvement projects, and evidence-based practice initiatives.
Maintain compliance with ACGME standards and institutional teaching requirements.

Working Conditions
Split practice environment between outpatient clinic and hospital-based CDU
Combination of scheduled clinic sessions and shift-based CDU coverage
May include evenings, weekends, holidays, and call responsibilities

Qualifications:
Qualifications
Required Education and Experience
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited institution
Completion of an accredited Family Medicine residency
Preferred Experience
Experience in both outpatient primary care and hospital-based or observation medicine
Experience or strong interest in graduate medical education
Experience in quality improvement, research, or clinical operations
Required Licensure/Certification
Current, unrestricted Texas Medical License (or eligibility)
Board Certified or Board Eligible in Family Medicine
Unrestricted authorization to work in the United States
