Full Job Description

Position Summary

At Signature Hospice, we believe exceptional hospice care extends far beyond the clinical. Our Social Workers are advocates, problem-solvers, encouragers, and trusted guides for patients and families during one of life's most meaningful journeys.

We're looking for someone who sees people—not paperwork. Someone who listens deeply, builds genuine relationships, and is passionate about helping patients and families navigate the emotional, practical, and personal challenges that often accompany serious illness.

This is an opportunity to make a lasting impact by serving patients with compassion, creativity, and initiative.

Essential Responsibilities

Complete comprehensive psychosocial assessments and contribute to individualized plans of care.
Provide emotional support, counseling, education, and crisis intervention to patients and families.
Help families navigate difficult conversations, advance care planning, and end-of-life decision-making.
Connect patients and caregivers with community resources, financial assistance, veterans' benefits, caregiver support, transportation, private-duty care, and other available services.
Advocate for patients and families, helping remove barriers to care and improve quality of life.
Collaborate closely with nurses, aides, spiritual care, volunteers, physicians, and leadership as part of the interdisciplinary team.
Maintain accurate, timely documentation in accordance with Medicare regulations and organizational standards.
Participate in bereavement support, memorial events, and community outreach as appropriate.

Qualifications

Master's Degree in Social Work (MSW) preferred; Bachelor's Degree in Social Work (BSW) considered in accordance with state and federal hospice requirements.
Current Oklahoma social work license, if applicable.
Hospice or healthcare experience preferred.
Excellent communication, organization, and critical-thinking skills.
Valid driver's license and reliable transportation.
