CMA for Telehealth Call Center - Work in Longview TX
NOTE: Not a remote work-from-home position. We are seeking experienced Certified Medical Assistants or for the position of “Care Coordinator”, within our Chronic Care Management (CCM) team.

Job Title: Chronic Care Management (CCM) (Care Coordinator)

Department: Care Management / Prime Care - Phynet - DCOL Reports To: CCM Program Manager / Clinical Supervisor

Position Summary: The Chronic Care Management (CCM) Care Coordinator is responsible for coordinating and managing care for patients with multiple chronic conditions. This role focuses on improving patient outcomes through ongoing telephonic and electronic communication, care plan development, patient education, and collaboration with providers and the interdisciplinary care team. The CCM Care Coordinator ensures compliance with Medicare CCM requirements while supporting continuity of care and reducing hospitalizations.

Key Responsibilities:

Conduct monthly CCM outreach to eligible patients via phone or other approved communication methods
Develop, implement, and update individualized, patient-centered care plans in collaboration with providers
Perform comprehensive patient assessments, including medical, psychosocial, and medication reviews
Provide education on disease management, medications, and preventive care
Monitor patient adherence to treatment plans and identify barriers to care
Coordinate services, referrals, and follow-up appointments with internal and external providers
Document all patient interactions and care coordination activities accurately and timely in the EMR
Track and document time spent on CCM services in accordance with Medicare guidelines
Ensure all CCM consent forms and required documentation are completed and maintained
Assist with care transitions, including hospital discharge follow-up when applicable
Identify high-risk patients and escalate concerns to providers as needed
Participate in quality improvement initiatives and team meetings

Productivity Expectations:

Complete 340 minutes of documented CCM time per day (or as adjusted based on program size and patient complexity)
Achieve a minimum of 20 minutes of documented CCM time per enrolled patient per calendar month
Maintain a monthly CCM completion rate of 100% for assigned patient panel
Maintain timely documentation of all patient interactions.
Ensure 100% of billed CCM encounters meet CMS documentation requirements
Follow up on hospital discharges within required timeframes when applicable
Keep care plans updated monthly for all active CCM patients
Maintain call quality standards, including patient engagement, education, and accurate documentation

Regulatory & Compliance Duties:

Maintain compliance with CMS guidelines for Chronic Care Management services
Ensure documentation supports billing requirements (minimum 20 minutes of non-face-to-face care coordination per month)
Protect patient confidentiality in accordance with HIPAA regulations

Qualifications:

Current (Other) Certified Clinical certification or license in the state of Texas
Minimum of 1–2 years of clinical experience; care coordination or chronic disease management preferred
Knowledge of common chronic conditions (e.g., diabetes, hypertension, COPD, heart disease)
Familiarity with EMR (ECW)systems and care management platforms
Strong organizational, communication, and critical thinking skills
Ability to work independently and manage time effectively
Familiarity with Excel

Preferred Qualifications:

Experience with Medicare CCM, TCM, or RPM programs
Bilingual (Spanish preferred, if applicable to patient population)
Experience in a primary care or outpatient setting

Work Environment & Schedule:

Office-based with frequent phone communication
Standard business hours Monday - Friday 8am to 5pm
