Job Summary
The Medical Case Manager is a social worker who provides intake, assessment, comprehensive care management services and education to clients of Careteam, Inc. and their caregiver(s) by implementing an individual action plan to meet the medical, financial, and emotional needs of the HIV+ individual. The goal of Medical Case Managers is to work with clients to achieve wellness and self-sufficiency through identification of service resources, education, and advocacy.
QUALIFICATIONS:
Must meet the *minimum in one of the 5 following ways:
Education and Experience
1. Hold a master’s degree from an accredited college or university in a human services field including, but not limited to, Social Work, Sociology, Health Education, Child and Family Development, Counseling, Psychology, or Nursing; or
2. Hold a bachelor’s degree from an accredited school of social work (BSW); or
3. Hold a four-year degree from an accredited college or university in a human services field or related curriculum including at least fifteen (15) semester hours in courses related to social work or counseling six (6) months of social work or counseling experience; or
4. Hold a four-year degree from an accredited college or university one (1) year experience in counseling or in a related human services field which provides experience in techniques of counseling, casework, health education, group work or community organization; or
5. Be a licensed Social Worker, or Certified Substance Abuse Counselor have two (2) years experience working in human service field.
Special Skills
- Understanding of highly confidential service provision and Code of Ethics as per the National Association of Social Workers.
- Bilingual fluency in Spanish and English preferred.
- Strong customer service focus and respect required.
- Effective verbal and written communication skills required.
- Organized, flexible and able to manage competing priorities required.
- Resourcefulness in problem solving required.
- Able to receive and follow through with delegated tasks and accountability required.
- High level PC skills, including but not limited to Microsoft Office software required.

- Valid and in good standing South Carolina driver’s license required.
- Personal transportation for travel to multiple work locations and client homes required.
- Meets Careteam Plus Employee Health Hepatitis B immunity program requirements.
- Meets Careteam Plus Employee Health Tuberculosis screening program requirements.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
To perform this job successfully, an individual must be able to perform each essential duty without accommodation or with accommodation without undue hardship to Careteam Plus.
- Assesses physical, psychological & social data from the client & family, medical records, health care team & other sources to formulate action plans initially, at specified/required intervals.
- Assesses community or agency service needs for IV, HIV/Substance Abuse and HIV/clients/families, as needed initially and with very required action plan.
- Assesses client satisfaction with service provided by interdisciplinary health team care, as needed or at least at 6-month review of action plan.
- Is a team player who collaborates with interdisciplinary health team members.
- Develops and maintains public relations with staff at clinic sites and other community agencies.
- Participates in ongoing public education campaigns in the service area to increase awareness of client/family needs.
- Serves as client's advocate and liaison between programs providing services to clients/families, to ensure the smooth flow of information and minimize barriers between service departments.
- Monitors adherence to the care plan in coordination with the client, client's family and health team while maintaining client confidentiality.
- Tracks clinic and other appointments and follows-up on missed appointments/concerns.
- Conducts case management functions in client home, at clinic sites, community agencies, residential programs and/or other sites, as needed.
- Identifies resources and links client/families with appropriate community agencies (medical, mental health, vocational rehab, Medicaid/Medicare, housing, insurance, Social Security, etc. ).
- Documents case management services in the client's case management record or other tracking or encounter form within required time frames after the encounter.
- As applicable, promotes syphilis testing to positive HIV and other clients.
- Provides ongoing emotional support to the client/families to assist in the coping with problems/challenges and utilizes complex professional services, as needed or with every 6 month appointments/service plan.
- Careteam staff work together as a team and help each other and patients/clients by performing other duties as assigned.
Pay: $36,358.00 - $61,760.00 per year
