Join PACE CNY as a Licensed Practical Nurse (LPN) at our Sally Coyne Center!
Under the supervision of the Clinic Manager and general direction of the Administrator/Director of Clinical Operations, the Clinic LPN is responsible for management of the medication program for the D&TC and PACE participants, including reordering of participants medications from pharmacy; conducting a variety of quality assurance functions per guidelines to monitor quality of services provided; completing variance reports as necessary; administering medications as ordered during day center hours of operation; and working closely with pharmacy personnel to ensure medication deliveries are accurate and timely. The Clinic LPN is also responsible for performing the highest quality of patient care and ensuring patient safety at all times according to accepted nursing practice standards.

Salary: $34.20 - $35.31 per hour

Responsibilities:
Job duties specific to this position:

Assists in clinic with participant care activities per schedule and performs other duties as assigned, including, but not limited to, daily blood sugar checks, injections, lab work, EKGs, vital signs, wound care, and nebulizer treatments, under the direction and supervison of the RN.

Performs participant care in a high quality manner, according to policies and procedures.

Observes the participant for specific signs and symptoms and reports changes or concerns to the RN in a timely manner.

Coordinates delivery of medications for community participants and ensures timely delivery to avoid interruption.

Communicates with pharmacy regarding renewals needed, reviews if renewal needed and initiates script for physician signature.

Updates medication management in Truchart and communicates with pharmacy.

Educates participants/caregivers about the purpose of the medication, correct dosage, etc.; answers questions when appropriate or directs participant/caregiver in medication administration; works in collaboration with home care case manager to reinforce education and training.

Checks MARS and worksheet daily to ascertain who receives medication for the day. Creates a MAR for participants requiring medication administration in day center. Updates the med remind list for day center.

Administers medications to participants on their day center days who require a nurse administration via a specific order to be given during day center hours. Daily checks pharmacy medication profiles against medical record medication list; makes corrections as necessary; faxes corrected profiles to pharmacy for dispensing.

Checks stock medications for expired drugs monthly per guidelines. Orders and maintains stock medications for the clinic as requested. Restocks Omnicell.

Reviews SNF medication orders for SNF residents with PACE medical record every month; makes corrections as appropriate so both records are accurate and complete.

Actively involved in Quality Improvement (QI) program; performs a variety of QI and compliance checks; i.e. verifies refrigerator temperature and outdated drugs checks, monitors the early refill processes, monitors the completion of MARS, etc.

Attends clinic and general staff meetings, in-service programs, etc. as required; may serve as clinic representative for special care planning meetings, etc.

Communicates with the pharmacy based on medication need changes, i.e. Hospitalization, traveling, address changes.

Demonstrates adherence to all compliance policies and procedures and the code of conduct.

Communicates expectations to employees regarding compliance-oriented behavior in the workplace.

Practices safety awareness, reports any possible safety/environment issues to supervisor, and adheres to safety policies and procedures.

Qualifications:
- Must be a graduate of an accredited school of nursing.

Must possess a current NYS license and registration as a LPN.

Minimum 1 year nursing experience and medication administration in acute or long-term care setting.

Demonstrated organizational, time-management and interpersonal skills.

Strong verbal and written communication skills.

Demonstrated ability to operate under busy, stressful conditions.

Ability to interact professionally and respectfully with family members or other support persons while providing care.

Proficient with MS Office and experience with Electronic Medical Record software. Ability to learn new software rapidly.
