Diverse Lynx LLC is actively seeking a dedicated Licensed Master Social Worker (LMSW) for a Full-Time Contract opportunity with a leading acute care hospital in New York City. Join a multidisciplinary Assertive Community Treatment (ACT) team providing recovery-focused behavioral health services to individuals living with serious mental illness.

Schedule: Monday – Friday | 9:00 AM – 5:00 PM

Hours: 35 Hours/Week

Job Summary

The LMSW will work within an Assertive Community Treatment (ACT) team, providing intensive, community-based behavioral health services to individuals with serious mental illness. This role involves conducting community visits, developing treatment plans, coordinating care across multiple healthcare systems, and supporting clients in achieving recovery and wellness goals.

Responsibilities

Provide direct behavioral health services in accordance with ACT program guidelines.
Conduct weekly community and office visits with assigned clients.
Manage a caseload of approximately 13–18 patients.
Complete psychosocial assessments, treatment plans, safety plans, and clinical documentation.
Coordinate care with hospitals, physicians, supportive housing, rehabilitation programs, and community agencies.
Deliver evidence-based interventions including:
Harm Reduction
Trauma-Informed Care
Motivational Interviewing
Stage-Wise Treatment
Monitor client wellness, medication adherence, and recovery progress.
Participate in multidisciplinary team meetings and treatment planning.
Maintain accurate documentation while ensuring compliance with organizational policies and regulatory requirements.

Qualifications Required

Master's Degree in Social Work (MSW).
Active New York State Licensed Master Social Worker (LMSW) license.
Minimum 1 year of LMSW experience.
SEL/SCR Certification required prior to start.
Strong knowledge of behavioral health and community-based mental health services.
Excellent communication, documentation, and case management skills.
