Full Job Description

Overview

Nexum HC is seeking a compassionate and dedicated Psychotherapist to join our growing team serving residents in skilled nursing and rehabilitation facilities in the East Moline and Silvis, Illinois area.

This is a full-time position offering a consistent caseload, flexible scheduling, strong administrative support, and the opportunity to make a meaningful impact in the lives of older adults and individuals receiving rehabilitative care.

$500 Sign-On Bonus Available*

Responsibilities

Conduct comprehensive assessments and diagnostic evaluations to identify mental health needs
Develop and implement individualized treatment plans based on client goals and clinical best practices
Provide individual psychotherapy and therapeutic interventions utilizing evidence-based practices
Facilitate group therapy sessions as appropriate
Maintain accurate and timely clinical documentation
Collaborate with facility staff and interdisciplinary care teams
Support residents experiencing depression, anxiety, adjustment concerns, grief, and other behavioral health needs
Promote emotional wellness and improve quality of life for residents

Qualifications

Master's degree in Social Work, Counseling, Psychology, or a related field
Active Illinois licensure (LCSW, LCPC, LPC, LMFT, LSW, or license-eligible clinician)
Strong clinical assessment and treatment planning skills
Excellent communication and interpersonal skills
Ability to work independently while collaborating with a multidisciplinary team
Experience working with older adults, healthcare settings, long-term care, or rehabilitation populations preferred but not required
IM+CANS certification preferred but not required

Location

Services are provided onsite at skilled nursing and rehabilitation facilities in East Moline and Silvis, Illinois. Facilities are located within a short driving distance of one another, minimizing travel time between locations.

*Employee must be hired into and maintain a full-time position for six (6) months to be eligible for the $500 sign-on bonus.
