Optimal Hospice is seeking a dynamic and compassionate individual to join our team as a Hospice Community Liaison. In this role, you will serve as a vital link between our organization and the community, fostering relationships with healthcare providers, facilities, and other community partners to ensure seamless access to our services and support.

Hospice Community Liaison's Responsibilities:

Develop and maintain relationships with healthcare professionals, including physicians, discharge planners, case managers, and social workers, to promote awareness of Optimal Hospice services.
Educate healthcare providers and community partners about the benefits of hospice care and palliative services.
Collaborate with internal teams to facilitate timely admissions and transitions of care for patients referred to hospice services.
Conduct outreach activities, including presentations, in-services, and networking events, to raise awareness of Optimal Hospice within the community.
Act as a resource for patients, families, and community partners seeking information about hospice services and end-of-life care options.
Represent Optimal Hospice at community events, health fairs, and professional meetings to enhance our visibility and reputation in the community.
Maintain accurate records of outreach activities, referrals, and communications in accordance with company policies and regulatory requirements.

Pay: $75,000-90,000

Qualifications:

Bachelor's degree in healthcare administration, marketing, business, or related field preferred.
Minimum of 1 year of experience in healthcare sales, marketing, or community relations, preferably in hospice or healthcare services.
Strong interpersonal and communication skills, with the ability to establish and maintain relationships with diverse stakeholders.
Knowledge of hospice regulations, Medicare/Medicaid guidelines, and healthcare industry trends is desirable.
Self-motivated and results-oriented, with the ability to work independently and as part of a team.
Proficiency in Microsoft Office suite and customer relationship management (CRM) software is preferred.
Valid driver's license and reliable transportation required.
