Full Job Description

Make a Meaningful Difference With a Predictable Schedule

This position is ideal for an RN who enjoys patient education, care planning, health coaching, and developing meaningful relationships with patients.

The clinic schedule is Monday through Thursday from 8:00 a.m. to 4:00 p.m, Friday from 8:00 a.m. to noon, and one Saturday a month, 8:00 a.m. to noon.

You will work in a collaborative outpatient environment, helping medically underserved Polk County residents manage diabetes and prediabetes and make sustainable improvements in their health.

Salary: $65,000 + benefits

Principal Duties and Responsibilities

Provide individualized diabetes and prediabetes education, health coaching, and ongoing patient support.
Monitor patient progress and provide regular follow-up, encouragement, and accountability.
Identify barriers that may prevent patients from following their care plans and help connect them with appropriate resources.
Participates in collaborative care model with referring APRNs and MDs.
Plans, coordinates, and executes DEEP (Diabetes Empowerment Education Program) classes, in person or virtually.

Qualifications/Skills

Excellent oral and written communication skills
Proficient with MS Office Suite, including Word, Excel, and Outlook
Thorough knowledge of modern office equipment, including multi-phone lines systems, fax, copiers, scanners, etc.

Education & Experience Requirements

RN credentials required
At least two years of medical experience
College degree preferred
Experience with EMR (electronic medical records) and medical terminology
Diabetes Education certification or background preferred.
