Full Job Description

Catholic Charities of Northern Nevada is seeking four Opioid Street Outreach Case Managers to provide grant-funded services to unsheltered individuals across Washoe County. These positions are based in Reno.

The Opioid Street Outreach (OSO) Case Manager conducts street-based outreach and intensive case management services for unsheltered individuals experiencing opioid use disorder (OUD) or at risk of OUD. This position is responsible for building rapport through consistent, in-person engagement; conducting screenings; facilitating referrals to substance use treatment, behavioral health services, and housing resources; and providing ongoing support to promote treatment engagement and housing stability. This is a grant-funded position at Catholic Charities of Northern Nevada under the OSO Program funded through the One Nevada Agreement.

Case Managers work under the supervision of the Opioid Street Outreach Supervisor/Coordinator and in coordination with community partners, licensed treatment providers, and Washoe County systems, including Coordinated Entry and HMIS.

This is a grant-funded position and contingent upon renewable grant funding beyond year one. Hours may vary for the OSO team and case managers may be scheduled to provide coverage on weekends, evenings, and/or provide on-call or emergency coverage.

ESSENTIAL FUNCTIONS AND BASIC DUTIES

Conduct proactive, street-based outreach in encampments, vehicles, and other locations not intended for human habitation to engage unsheltered individuals experiencing OUD or at risk of OUD.
Build rapport through consistent, trauma-informed, and harm reduction–oriented engagement, recognizing that trust develops over time.
Maintain an active caseload of 15–18 participants and conduct a minimum of 20 face-to-face outreach contacts per week, as required by the County.
Complete required brief screenings in HMIS to determine program eligibility and document outreach contacts, services, and outcomes accurately and in a timely manner.
Coordinate referrals to behavioral health and substance use treatment providers to determine appropriate levels of care, including residential, inpatient, or outpatient treatment.
Facilitate warm handoffs to licensed substance use treatment providers, behavioral health providers, and other community services to support successful service connection.
Assist participants with transportation planning and accompaniment to assessments, treatment intakes, medical appointments, and housing-related meetings as needed.
Develop and maintain individualized, person-centered case plans with SMART goals addressing treatment, housing, health, income, and stabilization needs.
Assist participants in obtaining critical documents, including government-issued identification, birth certificates, Social Security cards, and disability statements.
Make housing referrals through Washoe County’s Coordinated Entry system, including participation in case conferencing and placement into the Community Queue.
Maintain ongoing engagement with participants during and after treatment to support transitions to stable housing and continued housing stability.
Provide tenancy support services once participants are housed, including regular face-to-face contact, landlord communication, and support with accessing benefits, healthcare, and income.
Participate in case conferencing, multidisciplinary team meetings, supervision, and required trainings.
Adhere to all program policies, safety protocols, confidentiality requirements, and documentation standards.
Collaborate with supervisors and team members to adjust outreach strategies based on field conditions, participant needs, and performance data.

MINIMUM QUALIFICATIONS

Education: Bachelor’s degree (BA/BS) in Social Work, Sociology, Psychology, Public Health, Behavioral Health, OR equivalent combination of lived experience and professional experience. Bilingual highly encouraged.

Experience: Minimum of three (3) years of experience working with individuals experiencing homelessness, substance use disorders or co-occurring behavioral health conditions. Street outreach experience preferred.

Certification: Peer Recovery Support Specialist (PRSS) certification is a bonus, if applicant is in recovery. Community Health Worker (CHW) certification or ability to obtain also a consideration.

Skills/Abilities:

Ability to work safely and effectively in unsheltered, street-based environments.
Strong understanding of harm reduction, trauma-informed care, motivational interviewing, and de-escalation techniques.
Ability to engage individuals who may be actively using substances, in crisis, or distrustful of systems with empathy, respect, and professionalism.
Strong documentation skills and ability to enter accurate, timely data into HMIS.
Ability to manage a caseload while meeting outreach and documentation benchmarks.
Strong organizational, time management, and problem-solving skills.
Ability to work collaboratively as part of an interdisciplinary team.
Proficiency with Microsoft 365 and related data systems.
Ability to maintain professional boundaries and adhere to confidentiality requirements.
Cultural awareness, sensitivity, and commitment to equitable service delivery.

WORKING CONDITIONS

This position is primarily field-based and involves regular work in unsheltered environments, including encampments, streets, vehicles, and other locations not intended for habitation. The work may involve exposure to individuals experiencing active substance use, mental health crises, and unpredictable conditions. The position requires the ability to walk for extended periods, transport participants when necessary, and work flexible hours to meet program needs and ensure adequate community coverage, including varied hours and scheduling for on-call or emergency coverage.

Pay: $25.00 - $29.00 per hour
