Full Job Description

Help Expand Access to Dementia Care and Caregiver Support

HealthSpan Partners is a growing multi-state healthcare organization participating in the CMS GUIDE Model, providing dementia care management and caregiver support services to older adults living with dementia and their families.

We are seeking an experienced Community Partnerships Manager to build and strengthen relationships with healthcare providers, senior service organizations, and community-based organizations. This individual will play a key role in increasing awareness of HealthSpan's services, developing referral partnerships, and helping more families access dementia care support.

This position is ideal for someone who enjoys relationship building, community outreach, healthcare networking, and working collaboratively to improve services for older adults and caregivers.

What You'll DoPartner Relationship Management

Develop and maintain strong relationships with healthcare providers, senior service organizations, and community-based organizations.
Serve as a primary point of contact for partner organizations.
Foster long-term partnerships through regular communication and engagement.

Community Outreach & Education

Educate organizations about dementia care, caregiver support services, and HealthSpan's participation in the CMS GUIDE Model.
Represent HealthSpan at meetings, networking events, community presentations, and industry conferences.
Promote awareness of available dementia care resources for older adults and caregivers.

Referral Network Development

Identify and engage new referral sources and community partners.
Strengthen existing referral relationships to support sustainable growth.
Collaborate with organizations to streamline referral and communication processes.

Partner Onboarding & Support

Assist with onboarding new partner organizations.
Provide ongoing support, education, and resources to ensure successful collaboration.
Help partners understand referral workflows and available services.

Strategic Growth

Identify opportunities to expand HealthSpan's presence in new communities and markets.
Collaborate with leadership to develop outreach initiatives and partnership strategies.
Track outreach activities and partnership development efforts.

Strong organizational skills and ability to manage multiple relationships simultaneously.
