Full Job Description

Position Summary

The Health Information Specialist is responsible for accurately scanning, indexing, and maintaining patient medical records within the electronic health record (EHR) system. This position ensures timely availability of patient information by processing incoming documents, managing electronic records, releasing medical records in accordance with HIPAA regulations, and supporting providers and clinical staff with efficient document management. The HIM Scanning Specialist serves as a key resource in maintaining the integrity, confidentiality, and accessibility of patient health information while delivering excellent customer service.

Essential Duties and Responsibilities

Receive, review, scan, and accurately index incoming medical documents into the electronic health record (EHR) system.
Process incoming faxed documents and assign them to the appropriate physician, advanced practice provider, nursing staff, or department.
Scan office-generated documents into patient records following office policies and established workflows.
Enter demographic information and other required data into the EHR system.
Upload reports, correspondence, dictation, laboratory documentation, and other patient-related records into the appropriate electronic chart.
Monitor delinquent charts and communicate with physicians and advanced practice providers regarding outstanding documentation.
Release medical records in compliance with HIPAA regulations, state and federal laws, and organizational policies.
Collect applicable fees for medical record requests according to clinic policy.
Maintain strict confidentiality and security of all protected health information (PHI).
Recognize and appropriately respond to urgent or emergent situations according to established protocols.
Cross-train in the Call Center and provide telephone coverage as needed.
Assist in onboarding and training new employees.
Establish positive working relationships with physicians, advanced practice providers, clinical staff, management, and patients.
Perform additional duties as assigned by leadership.

Required QualificationsEducation

High School Diploma or GED required.

Experience

Minimum of six (6) months of medical office experience preferred.

Certifications

HIPAA Certification preferred but not required.

Knowledge, Skills, and Abilities

Strong attention to detail with the ability to maintain a high level of accuracy.
Ability to prioritize work and manage multiple tasks efficiently.
Knowledge of electronic health records (EHR/EMR) and electronic document management systems.
Proficiency with Microsoft Office applications, Windows-based software, and multi-page document scanners.
Excellent keyboarding and computer skills.
Strong verbal and written communication skills.
Ability to work independently while contributing effectively as a team member.
Strong organizational and problem-solving abilities.
Ability to adapt to changing priorities in a fast-paced medical office environment.
Professional communication with patients, providers, coworkers, and management.
Commitment to maintaining confidentiality and protecting patient information.

Physical Requirements

Ability to sit for extended periods while performing computer work.
Frequent use of hands and fingers for keyboarding, scanning, and data entry.
Occasional standing, walking, bending, reaching, and lifting office supplies or medical records.
Ability to lift and carry up to 25 pounds.
Ability to operate office equipment, including computers, telephones, printers, fax machines, and high-volume scanners.

Work Environment

This position is performed in a professional healthcare office environment. The Health Information Specialist regularly interacts with physicians, advanced practice providers, clinical staff, administrative personnel, patients, and outside healthcare organizations. Success in this role requires accuracy, confidentiality, strong organizational skills, excellent customer service, and the ability to manage multiple priorities while maintaining the integrity of the electronic health record.
