We are seeking a dedicated Registered Nurse (RN) who is passionate about making a real difference in patients’ lives. If you value autonomy, strong patient relationships, and meaningful community impact, this is the role for you.

What You’ll Do

Perform comprehensive in-home patient assessments
Develop and manage individualized care plans
Provide skilled nursing care including:
Wound care
IV therapy and injections
Catheter and ostomy care
Chronic disease management
Monitor patient progress and adjust care as needed
Educate patients and families on medications, nutrition, and self-care
Collaborate with physicians, case managers, and interdisciplinary teams
Complete accurate and timely EMR documentation
Ensure compliance with HIPAA and all regulatory standards

What We’re Looking For

Active Massachusetts RN license
Minimum 1 year of nursing experience (home health preferred)
Strong clinical judgment and assessment skills
Experience with wound care and chronic condition management
Ability to work independently and manage a flexible schedule
Reliable transportation and valid driver’s license
EMR/EHR experience preferred
