Job Description: Associate Case Manager
Compensation: $60,000-$70,000

Job Overview

The Associate Case Manager will have the opportunity to join a growing, innovative company focused on creating opportunities and connections for adults with intellectual and/or developmental disabilities (I/DD) to experience a rich meaningful life. The Associate Case Manager will report to the Director of Case Management. They will manage a caseload of their own clients in addition to supporting the Director of Case Management with high touch clients.

Responsibilities and Duties:

Manage a caseload of clients and support colleagues with their clients; including but not limited to:

Identify appropriate volunteer, internship and employment opportunities
Build and/or strengthen community connections (programming, friendships, relationships)
Build, grow, and sustain the client’s team of support; family, friends, staff and professionals
Visit the client in their home and participate in weekly commitments where welcome
Provide executive functioning and social coaching
Advocate for clients’ medical needs and coordinate and join clients at doctor’s appointments
Manage reimbursements with fiscal intermediaries and join life plan meetings
Support families and the team in the following areas:
Parent and family coaching
Housing options and roommate matching
Future Planning: document and implement a Letter of Intent for long term support into the future
Benefits Maximization and OPWDD/Self Direction Support

Other responsibilities include:

Communicate clearly and effectively with clients and their families in an ongoing manner
Write proficient case notes and provide thorough documentation utilizing EHR Your Way
Attend professional development training sessions
Attend individual and group supervision

Qualifications and Characteristics of the Ideal Candidate:

Masters Degree in Social Work, Mental Health Counseling or other related fields, LMSW preferred
LCSW supervision provided for clinical hours
Strong written and verbal communication skills
Passion for working with/advocating for adults with I/DD and their families
Belief in a person-centered approach that promotes a dignified and meaningful life for people of all abilities
Ability to develop relationships with professionals in the field and create new options for adults with I/DD
A background in state benefits and entitlements preferred (OPWDD, Self Direction, Medicaid and Medicare)

Job Type: Full-time

Pay: $60,000.00 - $70,000.00 per year
