JOB DESCRIPTION

Job Summary

Provides Speech Therapy services for residents/clients across Bethesda’s care settings, including the Skilled Nursing Facility, Home Health, and Outpatient programs, including evaluation, treatment, development, and execution of therapy plans, and completion of all required documentation and communication.

Job Relationships

· Reports to: Director of Rehabilitation

· Internal: SNF Administrator, Home Health and Hospice Director, Director of Nursing, Social Services, Human Resources, Accounts Payable, Billing.

· External: Physicians, Medical Director, MDH personnel, CMS surveyors, hospital discharge planners, referral sources, patients and families, and the general community.

Hours of Work: Full-time or Part-time.

Dress Code: Clean, fitted scrubs in solid colors or simple pattern.

Essential Requirements of Work: (Minimum qualifications necessary to function at full productivity.)

1. Education:

a) Bachelor’s degree from an Accredited Speech Therapy Program

b) Current accredited MN Speech Therapist License

2. Experience:

a) One to three years of prior experience in a rehabilitation setting, such as acute care, inpatient rehab, outpatient clinics, SNFS, long-term care, assisted living, memory care, or home health.

b) Knowledge of State and Federal regulations.

3. Personal:

a) Highly developed communication skills, both oral and written..

b) Self-motivated with strong attention to detail.

c) Strong problem-solving and decision-making skills.

d) Ability to adapt to various work settings and changing priorities.

e) Ability to work varying hours, including weekends and holidays.

4. Job Responsibilities:

a) Assess & Diagnose Communication Disorders: Evaluate patients' speech, language, voice, fluency, cognitive, and swallowing abilities; conduct screenings at regular intervals to determine need for intervention and treatment.

b) Develop & Implement Treatment Plans: Create personalized therapy programs to improve the functional deficits of the patient.

c) Provide Speech & Language Therapy: Conduct exercises and interventions to improve communication and speech abilities.

d) Educate & Support Patients, Families & Staff: Instruct patients, families, and staff in appropriate speech-language procedures and home programs to ensure carryover of therapeutic goals into daily activities; provide education on communication strategies and techniques to improve quality of life.

e) Monitor Progress & Adjust Treatment: Track patient improvements and functional outcomes; document assessment results, treatment progress, and follow-up; modify therapy plans as indicated to achieve established goals per physician orders.

f) Develop Community-Based Programming: Create appropriate home or community programming to maintain and enhance patient communication and swallowing performance in their own environment; participate in discharge planning.

g) Collaborate & Education Teams: Work with doctors, teachers, and therapists to ensure comprehensive support for patients.

h) Maintain Professional Standards: Adhere to facility policies, infection control guidelines, safety standards, and universal precautions; pursue continuing education and professional development; provide in-service training to care team members; maintain sound clinical judgment.

Physical Demands of the Job:

1. Standing and walking required for extended periods.

2. Occasional kneeling, stooping, squatting, twisting, and bending.

3. Sitting required for documentation, administrative, and computer-based tasks.

Professional and Personal Growth:

1. Report for and leave the work area on a timely basis.

2. Participate in the annual performance evaluation process.

3. Participate in all required in-service programs at Bethesda.

4. Maintain professional conduct and a positive, collaborative attitude toward staff, patients, and visitors.

5. Be willing to learn and adapt to meet the evolving needs of the organization.

Pay: $40.00 - $50.00 per hour
