CommonHealth Recovery, a growing integrated behavioral health and primary care outpatient treatment center, is opening a new Lexington location and hiring a Nurse Practitioner to split time between the new Lexington clinic and our established Frankfort clinic. This is a direct clinical role working with a multidisciplinary team across two central Kentucky locations.
Schedule & Locations
Monday through Thursday, 8:00 a.m. to 5:30 p.m. (36 clinical hours, 38 scheduled). Fridays off.

CommonHealth Recovery is a Kentucky-based outpatient organization reshaping how integrated care is delivered across the Commonwealth. Behavioral health, addiction treatment, and primary care live under one roof, giving patients a single team that treats the whole person. Licensed as both an AODE and DUI treatment facility, we serve Medicaid, Medicare, and commercial patients across counseling, IOP, MAT, psychiatry, primary care, and addiction medicine programs.
Position Summary
Provide Medication-Assisted Treatment (MAT) for patients with opioid, alcohol, and other substance use disorders, alongside primary care addressing acute, chronic, and preventive needs, including co-occurring medical conditions. This position reports to the Medical Director. Dual-certified PMHNPs will also provide psychiatric evaluation, medication management, and psychotherapy.
Clinical Responsibilities
- Addiction medicine: evaluate and treat opioid use disorder, alcohol use disorder, and other substance use disorders using medication-assisted treatment. Initiate and manage oral buprenorphine and naltrexone, and administer long-acting injectables including Sublocade, Brixadi, and Vivitrol. Conduct SUD assessments using standardized instruments (e.g., COWS for opioid withdrawal), query KASPER at intake and every three months, and educate patients on treatment options, risks, and benefits.
- Primary care: comprehensive health assessments, chronic disease management (diabetes, hypertension, hepatitis, cardiovascular disease), preventive care, lab and diagnostic interpretation, and medication management.
- PMHNP (if dual-certified): mental health evaluation and diagnosis, GAD-7 and PHQ-9 screening, medication management, psychotherapy, and 48-hour chart closure.
- Integrated care: collaborate with in-house case managers, behavioral health providers, and recovery support staff; refer to external specialty services as clinically indicated; participate in multidisciplinary case reviews and quality improvement.
- Compliance and documentation: maintain documentation supporting professional billing standards and comply with DEA, SAMHSA, and KBML regulations, including 201 KAR 9:310.
Required Qualifications
- Active Kentucky APRN license
- FNP or AGNP-PC board certification (ANCC or AANP)
- Active DEA registration with buprenorphine prescribing authority
- Prior EMR experience
- Ability to pass background check and drug screen; not on any Federal Exclusion List
Preferred Qualifications
- Dual PMHNP certification with at least one year of post-certification experience
- MAT experience (buprenorphine, naltrexone)
- Experience with co-occurring disorders and Medicaid populations
