Community Health Worker (CHW) – Outreach & Healthcare Access Support

About The Therapeutic Play Foundation

The Therapeutic Play Foundation (TPF) is a Los Angeles–based nonprofit organization focused on community wellness, prevention, healthcare access, and culturally responsive engagement. Through our Brain Well-Being Model® and Collective Care in Action® framework, TPF provides ARTGard™ wellness programming, Enhanced Care Management (ECM), healthcare access support, and community-centered outreach initiatives designed to strengthen resilience and improve access to supportive services.

TPF’s programming integrates wellness education, art, gardening, mindfulness, play, healthcare access engagement, and community partnership development to support individuals, families, and underserved communities throughout Los Angeles County.

Position Summary

The Community Health Worker (CHW) supports outreach, engagement, healthcare access coordination, ECM enrollment support, wellness programming, and community-based client engagement activities throughout Los Angeles County.

CHWs play a critical role in helping individuals and families connect to healthcare services, wellness resources, community supports, and TPF programming through outreach, relationship-building, follow-up communication, field-based engagement, and healthcare access support.

This role is highly engagement-focused and includes:

outreach communication,
client follow-up,
enrollment support,
community engagement,
Wellness Lab participation,
field-based outreach,
healthcare access coordination,
and ongoing relationship-building with assigned clients and community members.

CHWs are expected to maintain professionalism, responsiveness, flexibility, and strong communication while supporting TPF’s CalAIM and community wellness initiatives.

Key ResponsibilitiesClient Engagement & Enrollment Support

Follow up with individuals, families, and community members who express interest in TPF programs, Wellness Labs, healthcare access services, ECM enrollment, or community wellness initiatives
Support prospective clients with understanding available services, eligibility requirements, and enrollment steps
Assist with outreach, engagement, and follow-up efforts related to:
ECM referrals,
Managed Care Plan referrals,
community outreach events,
Wellness Labs,
popup wellness initiatives,
and healthcare access campaigns
Help individuals complete enrollment forms, gather required documentation, and navigate onboarding processes
Maintain timely and professional follow-up communication with prospective and enrolled clients
Support transitions-of-care outreach for individuals recently discharged from hospitals, shelters, residential placements, or other systems of care

Outreach Communication & Auto-Dialer Responsibilities

Required to answer and respond to outreach calls directed through TPF’s outreach communication and auto-dialer systems during assigned shifts and outreach hours
Conduct outbound engagement and follow-up calls to support:
client outreach,
appointment reminders,
enrollment engagement,
healthcare access support,
wellness event participation,
and referral coordination
Support outreach communication through phone calls, text messaging, email communication, and virtual engagement platforms
Maintain professionalism, responsiveness, and accurate documentation during all outreach communication activities
Support engagement efforts designed to reduce barriers to care and increase participation in TPF’s ECM and wellness programming

Community Outreach & Wellness Engagement

Participate in community outreach events, Wellness Labs, Community Play Dates, popup wellness initiatives, healthcare access events, food distributions, and resource fairs
Support engagement activities focused on:
healthcare access,
wellness education,
self-awareness,
emotional regulation,
coping skills,
and community connection
Assist with setup, participant engagement, registration support, and outreach logistics during events and wellness activities
Help distribute:
wellness kits,
backpacks,
diapers,
hygiene supplies,
and other outreach materials during community engagement activities
Support outreach and engagement efforts within schools, churches, laundromats, libraries, hospitals, and other trusted community spaces

Care Coordination & Community Support

Support assigned clients with linkage to:
healthcare providers,
behavioral health referrals,
wellness resources,
housing/community support resources,
food resources,
and other supportive services
Assist Lead Care Managers (LCMs) and outreach leadership with engagement and follow-up activities for assigned clients
Help identify barriers impacting healthcare access, engagement, or wellness participation
Maintain timely documentation and communication regarding client outreach activities and engagement efforts
Participate in interdisciplinary meetings, trainings, and collaborative outreach activities

Schedule, Availability & Field Expectations

CHWs are expected to maintain a minimum of three (3) in-person workdays per week based on assigned outreach activities, field assignments, Wellness Labs, healthcare access initiatives, meetings, and community engagement needs
Remaining workdays may include remote assignments; however, CHWs must remain available and responsive for additional in-person support activities if operationally needed
Assignments may include:
in-home visits,
field-based outreach,
community-based client engagement,
school/community meetings,
Wellness Labs,
popup outreach activities,
healthcare access events,
and in-person support for assigned clients throughout Los Angeles County
When not assigned to in-person activities, CHWs may be assigned:
phone outreach,
auto-dialer outreach shifts,
text and email engagement,
enrollment follow-up,
documentation tasks,
virtual meetings,
trainings,
professional development activities,
outreach coordination,
and administrative support assignments
CHWs are expected to maintain timely communication, responsiveness, professionalism, and accurate documentation across both in-person and virtual assignments
Flexibility and adaptability are essential due to the community-based nature of TPF programming and outreach operations

Preferred Qualifications

High school diploma required; associate degree, CHW certification, or related education preferred
Experience in:
community outreach,
healthcare engagement,
customer service,
social services,
case management support,
behavioral health,
or nonprofit/community work preferred
Strong communication, engagement, and relationship-building skills
Comfortable working in community settings and engaging diverse populations
Ability to maintain professionalism while supporting high-need populations
Experience using phones, text communication, email systems, Google Workspace, and outreach documentation systems preferred
Bilingual abilities strongly preferred
Reliable transportation and willingness to travel throughout Los Angeles County preferred

Compensation

Full-time hourly position
Anticipated compensation range: $22–$28 per hour, based on experience, reliability, outreach experience, engagement skills, and community-based work experience

Organizational Philosophy
