Experienced Licensed Practical Nurse (LPN) Home Care

Experience in Vent, Tracheostomy and G-tube

LPNs Starting at $420 a day!

Provide one-on-one care with one patient at a time in a calm home setting.

Flexible 12-hour shifts available. We are hiring for both pediatric and adult cases, including G-tube and ventilator support.

High Standard Home Care is a licensed Home Health Care Agency dedicated to providing compassionate, professional care to the elderly, individuals with disabilities, adults, and pediatric patients. Our team makes a difference in people’s lives every day by offering the support they need to age safely and comfortably in their homes.

We are seeking compassionate and dedicated LPNs to provide one-on-one care through our Private Duty Nursing (PDN) program. Unlike hospital or nursing home settings, you’ll care for one patient at a time in the comfort of their home, allowing for better focus, stronger relationships, and a lower-stress work environment.

In this role, you will supervise patients, assist with medications, and provide specialized care such as G-tube and ventilator management when required. This is a 1099 contractor position with flexible scheduling options, including 12-hour shifts, offering excellent earning potential in a supportive, low-stress environment.

What You’ll Do

Provide nursing care for PDN cases (children and adults)
Take vital signs, administer medications, and perform wound care
Supervise patients in their home to ensure safety and comfort
Assist with daily living activities, including bathing, dressing, eating, and grooming
Manage specialized care needs, including G-tube and ventilator support (training available if needed)
Monitor and document changes in patient condition, reporting promptly to the care team
Educate patients and families on health practices and care routines
Collaborate with physicians, care managers, and other healthcare professionals
Maintain accurate documentation and records in compliance with NYS and agency standards

What We’re Looking For

Current NYS LPN License (required)
Active malpractice insurance
Valid CPR certification
Previous PDN experience preferred (ventilator, G-tube, pediatric, or adult care)
Strong clinical judgment and critical thinking skills
Excellent communication and customer service skills
Organized and reliable, with the ability to track visits and follow-ups
Proficiency in documentation and electronic charting systems
