Position Title: Medical Records Clerk

Location: Carolyn Downs Family Medical Center in Seattle, WA (On-site)

Employment Type: 22 hours per week

Union: Yes

Salary: $22.74 – 33.95 per hour, payable bi-weekly. The salary scale for this position is between steps 0 and 19, depending on experience. This is a non-exempt, 22 weekly hours. This is a union position. Applicants must pass an extensive background check. Seattle Roots Community Health is a not-for-profit organization that operates two primary care clinics, an After-Hours Clinic, two School-Based Health Centers, a low barrier Medication-assisted treatment clinic and a Dental Clinic. Our Carolyn Downs Family Medical Center was founded in 1970 in Seattle's Central District. Our Country Doctor Community Clinic was established in 1971 by residents of the Capitol Hill neighborhood.

Job Summary

Through efficient maintenance of patient's medical records, you can play a significant role in facilitating the delivery of high-quality healthcare services to the patients of Country Doctor Community Health Centers. Your clerical support to the medical and outreach staff can make a meaningful difference in the lives of those in need.

Job Responsibilities

Medical Records Management:

Organize and maintain both electronic and physical patient records in accordance with clinic policies and legal requirements.
Ensure that all patient records are complete, accurate, and up-to-date.
Update and file patient charts, test results, medical histories, and consent forms.
Retrieve medical records for healthcare providers and staff in a timely manner to support patient care.

Compliance and Confidentiality

Ensure compliance with HIPAA regulations, maintaining patient confidentiality at all times.
Review medical records to ensure that they meet legal, ethical, and clinic-specific requirements.
Securely dispose of outdated medical records or records no longer needed in accordance with clinic protocols.

Data Entry and Electronic Health Records (EHR):

Enter patient data, medical histories, diagnoses, and treatments into the Electronic Health Records (EHR) system.
Ensure the accuracy and timeliness of data entry in EHR systems.
Assist in troubleshooting EHR-related issues, ensuring proper functioning and system updates.

Patient Records Requests and Communication:

Process requests for patient medical records from patients, authorized individuals, and other healthcare providers.
Communicate with patients to gather necessary information for medical record requests.
Respond to inquiries regarding the status of medical records requests.
Prepare and send medical records to insurance companies, referring physicians, or specialists as required.

Record Audits and Quality Control:

Conduct regular audits of medical records to ensure completeness and accuracy.
Work with healthcare providers and staff to resolve discrepancies or missing information in patient records.
Support clinic audits and quality improvement efforts related to medical records management.

Administrative Support:

Assist with scheduling patient appointments and organizing referrals for specialized care.
Perform general office duties such as answering phones, faxing, and filing paperwork.
Maintain and update paper and electronic filing systems for patient records.

Essential Functions:

Frequent sitting, standing, and use of a computer and telephone.
Occasional bending, stooping, and lifting (up to 25 pounds).
Adequate hearing, vision, and manual dexterity to operate office equipment (e.g., printers, fax machines).
Regular and reliable attendance to ensure smooth office operations.

Department Support & Flexibility:

Provide cross-coverage for other Health Information Management (HIM) teams and administrative functions as needed.
Assist with special projects, audits, record requests, and department initiatives.
Adapt to changing priorities and operational needs to support efficient medical records management, regulatory compliance, and continuity of patient care.

Training, Teamwork and Collaboration:

Support onboarding of new staff by demonstrating medical records workflows, document management processes, and best practices for maintaining accurate patient records.
Work with supervisors and managers to identify training needs and provide feedback on workflow improvements.
Collaborate with clinical and administrative teams to ensure timely processing, filing, scanning, indexing, and retrieval of patient records while supporting quality improvement initiatives.
Participate in staff meetings, department huddles, and ongoing training activities to maintain knowledge of policies, procedures, and regulatory requirements.

Qualifications:

- Previous experience in medical records management or a healthcare administrative role preferred.
- Familiarity with Electronic Health Records (EHR) systems (e.g., Epic, Cerner, etc.)
- Strong understanding of medical terminology and record-keeping practices.
- Excellent organizational skills with the ability to manage multiple tasks and priorities effectively.
- Strong attention to detail and accuracy in data entry and record management.
- Excellent communication skills, both written and verbal, for interacting with patients and healthcare providers.
- Ability to maintain confidentiality and adhere to HIPAA regulations at all time.
- Strong organizational skills with the ability to manage patient records in a fast-paced environment.
- Ability to work independently as well as part of a team.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook).
- Ability to handle sensitive information with discretion and professionalism.

Education & Training:

High School Diploma or GED equivalent required.
Completion of a Medical Records or Health Information Management course/program preferred.
Certification as a Medical Records Technician (CMRT) or Health Information Technician (RHIT) is a plus.
CPR or First Aid certification may be required depending on clinic policies.
