Full Job Description

Community Care Network is looking for a Developmental Services Consulting Nurse (RN) to join our team!

The Developmental Services Consulting Registered Nurse (RN) is responsible for the management and oversight of individual’s health care needs. The DS Consulting RN advocates for health supports that reflect the individual’s goals, preferences, and personal history in collaboration with individuals, families, guardians, case managers and the overall Team. The DS Consulting RN supports team planning, incident response, and transitions in care, in addition to promoting preventative health practices, managing chronic conditions, and facilitating access to primary and specialty care. The DS Consulting RN provides training and ensures all delegated tasks comply with the Vermont Board of Nursing standards, ensuring accountability and clinical integrity.

Developmental Services Consulting Nurse (RN) Responsibilities:

Assess the individuals health and establish nursing diagnosis, related to the individuals goals and health care needs.
Develop and implement medical and healthcare strategies, including nursing interventions.
Delegate appropriate nursing tasks to qualified staff.
Evaluate patient responses to interventions and oversee step-down care.
Collaborate with healthcare professionals to support high-quality care and management of chronic conditions.
Complete Emergency Fact Sheets at least annually and update as needed.
Maintain and update Special Care Procedures annually or as changes occur.
Monitor and verify accurate medication administration by staff and contractors.
Provide training and education on health conditions and related medical topics.
Ensure team compliance with Health and Wellness Standards, Medicaid guidelines, and state and federal regulations.
Review and interpret medical records to support quality of care and quality of life.
Document assessments, communications, and clinical recommendations in a timely, compliant manner.
Ensure service delivery and billing meet agency productivity and fee-for-service standards.
Conduct nursing assessments of psychiatric status to support medication management.
Consult with care teams before and after medical appointments to ensure continuity and quality of care

Developmental Services Consulting Nurse (RN) Qualifications:

Valid Vermont State Licensed Registered Nurse (RN).
One year of nursing in the field of Intellectual and Developmental Disabilities Services preferred.
Proficiency with computers, experience entering data into electronic medical records (EMR) program.
Ability to work with others in a positive, responsible manner.
Strong writing and organizational skills in order to receive and convey information in efficient and effective manner.
Ability to maintain confidentiality.
Flexibility, dependability, and ability to work independently.
Commitment to community inclusion and individual choice.
Able to flex schedule as needed.
Must have a valid driver’s license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
Successful completion of a criminal history background check after hire
