Part-Time Nurse Practitioner – Nephrology (Hemodialysis)

Bensonhurst & Dyker Heights, Brooklyn, NY

Park Avenue Nephrology is seeking a compassionate and motivated Nurse Practitioner or Physician Assistant to join our growing private nephrology practice on a part-time basis (1–2 days per week) caring for patients receiving in-center hemodialysis in the Bensonhurst and Dyker Heights areas of Brooklyn.

This position is ideal for an experienced NP looking for flexible scheduling while practicing high-quality nephrology in a supportive physician-led environment.

Position Overview

The Nurse Practitioner/Physician Assistant will provide weekly follow-up care for established chronic hemodialysis patients in outpatient dialysis facilities under the supervision of our nephrologists.

Responsibilities include:

Perform focused weekly dialysis rounds on established ESRD patients
Evaluate volume status, blood pressure, dialysis adequacy, and access function
Review laboratory studies and assist with management of: Anemia, CKD-mineral bone disease, Electrolyte disorders, Nutrition
Identify patients requiring physician evaluation or hospitalization
Coordinate care with dialysis nurses, dietitians, social workers, and vascular access providers
Document visits in the dialysis electronic medical record
Communicate closely with supervising nephrologists regarding patient issues

Schedule

Flexible
1–2 days per week
We are open to hiring:
one provider for two days per week, or
two providers working one day each.

Qualifications

Required

Licensed Nurse Practitioner or Physician Assistant in New York
DEA registration (or ability to obtain)
Excellent clinical judgment and communication skills
Ability to work independently while collaborating with supervising physicians

Preferred

Nephrology, internal medicine, hospital medicine, cardiology, or primary care experience
Familiarity with dialysis EMR documentation
