Full Job Description

Job Summary
We are seeking a dedicated and experienced Bilingual PHC (Primary Home Care) Supervisor to oversee and ensure the quality of in-home care services, including training, supervising staff, and coordinating with clients and other healthcare professionals. The role also involves ensuring compliance with regulations and maintaining accurate records.

Duties

Client Care: Conduct home visits to assess client needs, develop care plans, and monitor the delivery of services.
Supervision and Training: Supervise and mentor staff, ensuring they are adequately trained and competent in providing in-home care services.
Documentation: Maintain accurate and up-to-date records of client assessments, care plans, and service delivery.
Communication and Coordination: Communicate effectively with clients, caregivers, and other healthcare professionals to coordinate care and address concerns.
Compliance: Ensure adherence to relevant regulations, guidelines, and agency policies related to PHC services, including HIPAA and quality standards.
Performance Improvement: Identify areas for improvement in service delivery and implement strategies to enhance the quality of care.
Foster a positive work environment that encourages teamwork and professional development.

Qualifications

High School Diploma or Equivalent.
Fluent in Spanish and English languages.
Proficient with computers. Microsoft Word and Excel and PDF.
Previous experience with Vesta EVV is a plus.
Previous experience in a medical office setting is a plus.
Good communication skills.
Good organizational and time management skills.
Must have and maintain a valid state driver's license.
