We build High-Value Referral Networks that connect specialists and primary care across Michigan, enabling smarter referrals, stronger care coordination, and measurable performance results for our partner practices.

Urology is one of our fastest-growing partnership areas. We are expanding our network of urology practices statewide as part of a value-based program with a major Michigan payer and we need a strong relationship manager to own and grow those partnerships.

This role is ideal for someone with a background in healthcare account management, provider relations, or business development who thrives on building long-term relationships and driving measurable results.

Role Overview

This is an account management role at its core. You will own a portfolio of urology practice relationships across Michigan and will be responsible for engagement, adoption, performance, and retention. You are the face of TRIARQ to your practices, and your success is measured by how well those partnerships perform.

A typical week might look like: a quarterly business review with a urology group in Grand Rapids, a check-in call with a practice manager in Lansing to review their referral scorecard, and a planning session with TRIARQ’s care management team to align on a practice that’s underperforming on network utilization. You will report directly to senior leadership within the Provider Partnerships team.

What you’ll own:

A defined portfolio of urology practice accounts across Michigan statewide.
Practice engagement and adoption of TRIARQ’s referral and care management platforms.
Quarterly business reviews that hold practices accountable to performance goals and deepen the partnership.
Scorecard delivery and performance conversations that translate data into action.
The day-to-day relationship with practice administrators, physicians, and clinical staff.
Feedback loops back to TRIARQ leadership on practice needs, barriers, and market intelligence.

Key Responsibilities

Manage and grow a portfolio of urology practice partnerships, serving as the primary relationship owner for each account.
Drive platform adoption across your accounts — referral tools, care management technology, and performance dashboards.
Lead quarterly business reviews, track performance against goals and identify opportunities to deepen engagement.
Deliver practice scorecards and translate performance data into clear, practice-friendly action plans.
Identify and resolve adoption barriers quickly, escalating internally when needed to protect the relationship.
Partner with care management, member experience, and marketing teams to coordinate support across your accounts.
Maintain accurate account records, activity logs, and engagement status in CRM.
Meet or exceed account engagement and adoption targets on a quarterly basis.

Qualifications & Experience

2–4 years in healthcare account management, provider relations, business development, or customer success.
Proven track record managing a portfolio of client or provider relationships and hitting engagement or adoption targets.
Experience working with physician practices, provider organizations, or specialty providers preferred; urology or MSK background a plus.
Confident presenter and communicator — comfortable leading business reviews with physicians and practice leadership.
Comfortable with EHRs, CRM, or care management platforms (or willing to learn).
Highly organized and self-managed, able to prioritize across a large account portfolio with minimal oversight.
Field-ready — comfortable spending meaningful time on-site with practice partners across Michigan.
