Salary Range: $5,943.91 - $8,575.40 per month

SUMMARY

The Health Program Analyst I (HPA I) will support the coordinated release of ODR clients from Los Angeles County Jail as a part of ODR’s ODR Housing Program - which is a jail-based diversion and permanent supportive housing program focused on the release of individuals with high mental health acuity from Los Angeles County Jails. Primary responsibilities include the in-custody, or in court, screening of clients’ substance use support needs, making recommendations for interventions, and making referrals to substance use referral resources – such as residential substance use treatment facilities. Additionally, this role will serve as a liaison to the ODR Central Release and RAD (Referrals Access & Data) teams for the coordination of client releases to community-based housing and services.

Currently, ODR has a hybrid work schedule with a combination of workdays in the office, in the field and remote. This may change at discretion of DHS.

ESSENTIAL FUNCTIONS

Coordinate and manage the jail release process for ODR clients requiring referrals to substance use resources in collaboration with other members of the ODRH Housing Pre-Release and Central Release Teams.
Collaborate with ODR physicians and social workers to ensure clients are appropriately assessed and treatment plans incorporate interventions to address clients’ substance use needs.
Collaborate with doctors, nurses, analysts, and clinicians to assess client overall needs and ensure that housing placements and Intensive Case Management Services (ICMS) assignments are secured prior to release.
Assist with the creation of new client profiles and maintain accurate client records.
Enter, track, and report release tracking data for monitoring and accountability.
Meet with clients in custody to orient them to the ODR program and prepare them for transition to community-based services.
Oversee and coordinate residential substance use disorder (SUD) treatment placements, including identifying and securing programs for clients who are court-ordered to residential treatment.
Develop and implement systems to track SUD service utilization, timelines to access, and effectiveness of interventions. Analyze data to drive performance improvement.
Act as a liaison to LA County Department of Public Health – Substance Use Prevention and Control (SAP-C), and to SAP-C’s network of community based providers in order to ensure effective collaboration, coordination of care, and resource availability for ODR clients.
Conduct jail, and court lock-up, visits to orient clients to the program and expectations upon release, assess housing location preferences and needs, gain consent for referral services, and triage changes in functioning that may impact level of care.

JOB QUALIFICATIONS

Option I : A Bachelor's degree from an accredited college or university in Social Work, Counseling, Health Care Administration/Management, Public Administration, Behavioral Sciences, Social Sciences, Business Administration, or a closely related field -AND- two years of direct professional experience working with persons experiencing homelessness, serious mental illness, or justice involvement, in the role of case manager, clinician, outreach worker, program manager, or similar.

A Master's degree from an accredited college or university in Social Work, Counseling, Health Care Administration/Management, Public Administration, Behavioral Sciences, Social Sciences, Business Administration, or a closely related field may be substituted for one year of the required experience in the analysis of mental health or public health programs. This is only applicable to option I.

Option II: Four years of direct professional experience working with persons experiencing homelessness, serious mental illness, or justice involvement, in the role of case manager, clinician, outreach worker, program manager, or similar.

Preferred Experience

Direct experience working with justice-involved populations and/or within systems of care (e.g., housing, mental health, substance use treatment).
A bachelor’s degree or higher from an accredited college or university, preferably in Public Health, Public Administration, Business Administration, Healthcare Administration, Healthcare Management, Biostatistics, Epidemiology, Social Work, Psychology, Community Development, Criminal Justice, Paralegal Studies, or a related field.
License or certification as a substance use counselor, or at least 1-year professional experience conducing substance use screening or assessments.
Experience coordinating discharge planning from carceral settings, hospital or healthcare settings, SUD treatment programs, or similar.
Proficiency in Microsoft Excel (data entry, tracking, and reporting).
Strong organizational skills and ability to collaborate across multidisciplinary teams.

Certificates/Licenses/Clearances

License or certification as a substance use counselor is preferred.
Successful clearing through the DHS Live Scan, Health Clearance and LASD Clearance process with the County of Los Angeles.
A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

Proficient skill set in using an array of Microsoft Office Suite software programs such as Word, Excel, PowerPoint, Access, Adobe Reader, One Note, Outlook, Publisher, Teams, Outlook, etc.
Able to multi-task and set workload priorities for time sensitive projects/tasks.
Ability to problem solve and make recommendations to processes, policies, etc.
Able to communicate with all levels of personnel, e.g., written, verbal, in a professional and concise/clear manner; ability to work within a project team and/or independently.
