SELF-DETERMINATION SERVICES BROKER MANAGER

Rate: TBD Based on Experience

Job Summary:

The Self-Determination Services (SDS) Broker Manager is responsible for educating and supporting individuals served by Community Living Services, and providing training and resources for families, Support Coordinators, Independent Support Coordinators, service providers, and other internal/external stakeholders on the Principles of Self-Determination. This will be a transitional role which will start with directly providing SDS Broker services, and then managing a department of SDS Brokers as the volume of referrals for this service grows. This position will serve as the liaison for all Self-Determination stakeholders and will be focused on advancing Self-Determination, increasing personal empowerment and control in the lives of individuals supported, and making the self-direction process less intimidating for individuals and their families/support circles. The SDS Broker role that falls under the umbrella of Medicaid funded Support Coordination. All SDS Support Broker services provided are to be outlined in each person’s Individual Plan of Service, authorized via a person-centered planning process and provided in coordination with the person’s existing Support Coordinator/ Case Manager and under the approval of CMH/PIHP funder regulations and procedures. This is a position that comes with the expectation of growth of the department / doing good work so new referrals keep coming in, and expansion into new regions based on reputation and promotion of the SDS Broker model of support.

Qualifications :

SDS Broker Manager must demonstrate competency in areas of SDS philosophy and processing. Minimum Bachelor Degree and one year of experience working with individuals with disabilities.

Preferred experience in identified areas of responsibility with ability to work collaboratively and ensure quality outcomes, satisfaction with services, and timely documentation of services as authorized in the Individual Plan of Service.

Duties and Responsibilities:

The duties described below will be provided directly by the Manager during the initial months on the job until the volume of SDS Broker referrals warrants posting a position to add to the department. Once positions are added to the team, this Manager role will transition to oversight and supervision of the direct service role. At the point of the transition, the below duties will transition from “provide” to “oversee the provision of.” It will be expected that the Manager represents Community Living Services in a leadership role and works to promote and expand the Broker department’s work.
Provides direct Medicaid-funded services aimed at helping people to acquire and develop skills necessary to self-direct through either face-to-face contacts or via video /telehealth contacts based on the preferences of people served.
Provides education and guidance on the principles and practice of Self-Determination (SD) to internal/external stakeholders.
Facilitates self-directed support (SDS) arrangements by explaining the self-directed budget process, employer responsibilities, related agreements/documents, and the roles of the Financial Management Service (FMS) Agency, funding agencies, and the person’s chosen providers.
Provide guidance about self-directed service options; alert the individual to self-directed service options/spending plan requests that require prior authorization or approval from funders before implementation.
Attends Spending Plan meetings, assists with Budget Worksheet review and signature obtainment.
Assists people in their Employer of Record roles by meeting with them routinely and as requested for support.
Completes Broker Progress Notes using collaborative documentation methods and signs document no later than 48 hours following the service date.
Attends PCP meetings as requested by people served, and assists Support Coordinators with Individual Plan of Services wording to align IPOS goals with Spending Plan requested purchases.
Acts as an advocate to get timely responses for the self-directed spending plan and budget requests.
Assists person to obtain other benefits and utilize generic community resources not otherwise covered by the Supports Coordinator (SC)/Independent Supports Coordinator (ISC), SC assistant, and/or other support brokers.
Acts as a liaison between the individual and the FMS agency, Agency with Choice, funders, and other stakeholders related to SDS arrangements.
Helps evaluate risks and make sure urgent needs are met by assisting the person to develop back up plans; Helps person to recruit workers and/or provide suggestions and advice about managing workers.
Provides support for individuals related to employee management activities; Advocates for any information, training, or assistance the individual needs or desires from their FMS agency, Support Coordinator, or SDS Administrator.
Assist with obtaining required signatures are on the self-directed documents/agreements.
Troubleshoots issues with SD authorizations and collaborates with SCs/ISCs, FMS Agencies, funders and providers for resolution.
Acquires knowledge of all CLS Broker functions and assists with those broker functions as determined by CLS’ business needs and the needs of the people we serve.
Assists people in understanding the Medicaid documentation and training requirements and Electronic Visit Verification System (EVV) when it is active in Michigan.
Assists with administrative functions, data collection, writing policies and procedures and other assigned duties as deemed appropriate for CLS business needs.
Other duties as assigned.

Training :

On-the-job training will be provided. Must complete mandatory training requirements on an annual basis.

Travel Requirement :

Periodic scheduled and non-scheduled travel is required in executing essential duties and responsibilities of this position. Employees must provide their own transportation. Maintenance of a valid driver’s license, an acceptable driving record, and vehicle registration and insurance in compliance with State of Michigan requirements are all continuing condition of employment. If driving a vehicle other than the one listed on file with the Human Resources Department, employee is responsible for ensuring the vehicle is registered, insured, and meets all other State of Michigan motor vehicle requirements and providing documentation as requested.

CLS policy restricts the use of an alternate driver without the express, prior written authorization of the Human Resources Director. A review of such requests will be based on the evaluation of HIPAA compliance issues, work restrictions, and other relevant factors.
