Full Job Description

The Medical Doctor/Family Practitioner provides compassionate, patient-centered primary care to individuals across the lifespan within a Federally Qualified Health Center (FQHC) setting. This role focuses on the prevention, diagnosis, and management of acute and chronic conditions through high-quality, evidence-based care. The physician collaborates with an interdisciplinary care team to ensure continuity of care, optimal patient outcomes, and adherence to FQHC standards and federal regulations. Full-time exempt physicians are expected to engage in quality improvement initiatives, care coordination, audits, and collaborative care activities.

Classification: Exempt, Full-Time (Salary) $250,000 - $270,000 annually. This position is classified as exempt under the Fair Labor Standards Act (FLSA) and is not eligible for overtime pay.

Essential Duties and Responsibilities:

Clinical Care

Conduct comprehensive patient assessments, including health history, physical exams, and diagnostic evaluations.
Provide comprehensive primary care services to patients across the lifespan, consistent with education, training, licensure, board certification, and scope of practice.
Develop individualized treatment plans and monitor patient progress.
Order, interpret, and communicate lab tests, imaging, and other diagnostics.
Provide preventive care, including immunizations, screenings, and patient education.
Manage chronic diseases such as diabetes, hypertension, asthma, and heart disease.
Prescribe medications appropriately and monitor for efficacy and side effects.
Refer patients to specialists or community services as necessary.
Document all patient encounters accurately and in compliance with clinic policy and legal requirements.
Participate in telehealth visits, in-person care, and urgent care coverage as scheduled.

Patient-Centered Care

Deliver culturally competent and respectful care to diverse patient populations.
Educate patients and families on treatment plans, medications, lifestyle modifications, and preventive measures.
Support patient engagement and shared decision-making.
Respond to patient concerns or feedback professionally and follow up as appropriate.

Collaboration and Teamwork

Work collaboratively with nurses, medical assistants, behavioral health providers, and administrative staff to coordinate care.
Support cross-coverage for providers as needed to ensure continuity of patient care.
Develop and maintain professional relationships with colleagues, including physicians, practice managers, nurse managers, office managers, and community providers.
Participate in care team meetings, case discussions, and care coordination activities as scheduled.

Quality, Compliance, and Professional Practice

Adhere to all FQHC regulatory requirements, HRSA guidelines, and organizational policies.
Participate in audits, quality improvement initiatives, and accreditation activities as scheduled.
Support performance on quality metrics, including HEDIS measures and Uniform Data System (UDS) reporting requirements.
Maintain accurate, timely, and legally compliant documentation in the electronic medical record (EMR), including order entry and care coordination.
Complete and close all patient encounter documentation within 72 hours of the patient visit, in accordance with organizational policy.
Report adverse events or incidents according to clinic policies and professional standards.
Ensure billing and consultation charges comply with clinic policy.
Maintain professional integrity and ethical standards in all clinical practices.

Professional Development

Maintain required licensure and board certification.
Participate in continuing medical education (CME) and stay current with clinical guidelines and best practices.
Support organizational mission, values, and standards of patient care.

*Performs other duties as assigned. Responsibilities may be modified or expanded to meet organizational needs.

Qualifications:

Education & Licensure

Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) required.
Current and unrestricted license to practice medicine in California.
Board certified or board eligible in Family Medicine or related primary care specialty.

Experience:

Minimum of four (4) years of clinical experience preferred.
Experience in community health centers or FQHCs strongly preferred.
Experience with utilization management, quality improvement, and physician practice management is a plus.

Skills, Knowledge, and Abilities

Commitment to patient-centered care and serving diverse populations.
Excellent interpersonal, verbal, and written communication skills.
Ability to work independently and collaboratively in a multidisciplinary healthcare team.
Knowledge of healthcare regulations, compliance standards, and clinical quality measures.
Proficiency with electronic medical records (EMR) systems, preferably Allscripts.
Ability to prioritize and manage multiple tasks effectively.
Strong problem-solving, critical thinking, and planning skills.

Computer & Equipment Skills

Proficiency in Microsoft Office (Word, Excel, Outlook) and internet-based applications.
Use of EMR systems for documentation, scheduling, and patient communication.
Ability to learn new software programs quickly.
Use of standard office equipment (fax, phone, scanner, copier) as needed.

Reasoning and Mental Ability

Apply critical thinking to assess patient needs, interpret complex information, and make informed clinical decisions.
Plan, organize, and prioritize work efficiently.
Evaluate situations and requirements to adjust care plans accordingly.
Project likely outcomes and take appropriate preventive or corrective actions.

Pay: $250,000.00 - $270,000.00 per year
