Full Job Description

DUTIES AND RESPONSIBILITIES:

* RESPONSIBLE FOR THE CONTROL OF MEDICATIONS

Monitor, label, store, and approve all incoming medications for clients before they are logged in and clients are permitted to take them. If any incoming medication is not approved (i.e., Narcotic, Alcohol, etc), the Nurse is responsible for locking it away until it can be removed from Elmhurst appropriately.
Return discarded or abandoned medications to the pharmacy, through Rx destroyer, or prescription drug takeback.
Review the monthly for accuracy, compliance, and appropriate use of each medication for each client.

CONDUCT QUALITY ASSURANCE PERFORMANCE PLAN MONITORING

Complete the Medication Compliance Audit Report Form to ensure client compliance with the medication regimen. Special attention should be paid to psychoactive [psychiatric] medication and to medication for diabetes, hypertension, and seizure disorders.
Ensure findings are within the EHR for the client and the client’s care team to address accordingly.

CONSULTATION AND COLLABORATION

Serve as a primary consultant to Executive leadership and clinical staff to ensure coordination, triage, referral, and recommendation for clients’ medical issues, problems, and/or concerns.
Consult with clinical staff about medical needs of clients to inform recommendations, referrals, and decisions.
Consult with the manager of the dietary department when needed to inform of food-based accommodations.
Liaison with external stakeholders e.g., pharmacy, laboratories, PCPs, TB testing, specialty clinics, mobile medical units, etc. to ensure coordination of care and access to PCU services.
Represent EHI in workgroups and forums e.g., MDHHS, DWIHN, etc.
Ensure community visibility through wellness events e.g., SBIRT, Prescription Drug Takeback, etc.
ACT/IDDT
Coordinate medical needs of clients with ACT/IDDT teams.
Participate in ACT team huddles as needed ensuring access to medications, care coordination, etc.
Attend ACT Nursing Consultations calls, ACT Forum meetings, etc.
Provide services to include LAI administration within the community e.g., client home, shelter, etc.
Provide on-call support as needed.
Service Delivery
Perform nursing assessment on all outpatient mental health clients and document within the EHR within 24 hours of service.
Conduct physicals on all residential SUD clients and document within the EHR within 24 hours of service.
Administer LAI injections on a routine basis to clients as well as oral, intramuscular, subcutaneous, or intradermal medications by physician’s order.
Provide basic nutritional counseling for clients with special dietary needs and referral to outside sources when appropriate.
Conduct didactic groups on health, medication, wellness, etc.
Make daily assessment of clients’ medical status i.e. medical instability when needed to ensure follow-up as necessary, and to inform appropriateness of placement for continued care or referral.
Document all client contact, outreach, engagement, progress, consultation, etc. within the EHR within 24 hours while ensuring appropriately authorized CPTs and NGBs.
Administer basic first aid within the educational scope (including CPR).
Training and Supervision
Provide documented supervision and performance evaluation to PCU staff e.g. RN, LPN, etc. to inform staffing decisions.
Participate in onboarding new staff as relevant to the role of the PCU to the staff position.
Other medical-related duties as deemed necessary to the function of the facility
Other duties as assigned

MINIMUM REQUIREMENTS – EDUCATION AND EXPERIENCE

Current licensure in the State of Michigan

Residential substance abuse treatment experience desired
Current CPR certification
No active warrants. Must have been discharged from all sentences including parole and probation for at least (5) years.
Must be able to pass LEIN check.

Pay: $43.58 - $52.48 per hour
