Company overview: Lumina Care is focused on unifying, coordinating, and managing care for geriatric patients in nursing facilities and at home. We offer a range of services to improve health outcomes and quality of life for patients, including afterhours telehealth, transitional care, chronic care, remote patient monitoring, behavioral health and telehealth psychiatry.

Job title: Community Care Manager

Purpose/Summary: We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Community Care Management team. The successful candidate will have a background in long-term care and enjoy working within a collaborative care team environment.

Key Responsibilities:

Conduct telephonic outreach to patients for care coordination, follow-ups, and health assessments.
Schedule virtual and in-person medical appointments as needed.
Assess and document patient-reported vital signs and health status remotely.
Perform medication reconciliation to ensure accurate medication management.
Develop, update, and implement personalized care plans that align with patient needs and healthcare goals.
Collaborate with interdisciplinary healthcare teams (physicians, nurses, specialists) to enhance patient care.
Maintain detailed electronic documentation to meet regulatory and Medicare compliance standards.
Utilize telehealth technology and electronic health records (EHR) systems efficiently.
In performing the Services, you shall follow all applicable laws and regulations.
[You represent and warrant to the Company that you currently hold, and will maintain during the term of this engagement, an unrestricted license to practice as an LPN, in those states in which Services may be rendered (as advised to you by the Company from time to time).]
As a Community Care Manager, you agree to comply at all times with the Company’s written policies applicable to your employment as a care manager, including without limitation, clinical, fraud & abuse prevention and HIPAA policies, as established and amended by the Company from time to time.

Credentials:

Active and unrestricted compact LPN or RN licensure is required.
Willingness to obtain additional state licensure as requested.
LPN diploma or certificate of academic completion required.
If an RN, an Associate Degree in Nursing is required or a Bachelor of Science in Nursing is preferred.

Qualifications:

A minimum of two years of experience as an LPN or RN is required.
Long-term care experience is preferred.
Technology proficiency is required.
Self-directed with the ability to work autonomously, but functions as a contributing member of the team.
Written and spoken English language proficiency is required

Working conditions:

A designated, secure, and private home workspace is required for this remote role to ensure compliance with HIPAA and company privacy and security policies. This workspace must support confidential patient communications and prevent unauthorized access to protected health information. Typing is required and you will be required to use Company approved virtual technology to visualize patients during visits. A personally owned computer used for this role must meet all company-mandated security and encryption requirements as defined by the IT team. Company-provided security software, including platforms for patient video consultations, may need to be installed to ensure the device remains compliant with organizational data protection standards. Periods of prolonged sitting or standing can be expected. Travel requirements could occur based on specific state licensure and leadership needs.

Compensation: $27-$35 per hour. Compensation is determined based on market range, experience, skills, education, and qualifications.
