Full Job Description

Join Encounter Telehealth, the leading rural behavioral health provider, as our Care Navigation & Reporting Coordinator for the Nebraska RHTP 4.4A program. This is a full-time, hybrid role supporting approximately 250 rural Nebraskans across 8 partner skilled nursing facilities and critical access hospitals.

This dual-function position splits time roughly 50/50 between hands-on patient navigation and clinical data collection / DHHS grant reporting. You're the person who connects patients to care - and the person who proves it's working.

Patient Navigation Responsibilities

Coordinate care for ~250 enrolled participants across 8 rural partner facilities
Manage closed-loop referral tracking from behavioral health screening through psychiatric evaluation and therapy
Support medication adherence and reconciliation in coordination with prescribing providers and pharmacies
Help patients navigate insurance, transportation, food access, and other social determinant barriers
Serve as point of contact for facility administrators, discharge planners, and nursing staff
Support PHQ-9 and GAD-7 screening implementation across partner facilities

Data Collection & DHHS Reporting Responsibilities

Compile and submit quarterly and final reports to Nebraska DHHS
Conduct and refresh community needs assessment data quarterly
Monitor grant performance measures: enrollment, screening completion, referral closure, rehospitalization
Validate clinical data from partner facilities against approved data sources
Work within data infrastructure maintained by Encounter's CTO team
Maintain working knowledge of 2 CFR Part 200 grant compliance requirements

Education & Credentials

Strongly Preferred: RN or LPN
Also Considered: LSW/LCSW, Certified Medical Assistant (CMA), Respiratory Therapist, Physical Therapist, or other licensed healthcare provider
Minimum: Bachelor's in Healthcare Administration, Social Work, Health Informatics, or related field with equivalent clinical experience

Experience Required

3+ years in care coordination, case management, patient navigation, or clinical role with documentation/reporting
Chronic disease (diabetes, CVD, COPD, CKD) or behavioral health experience strongly preferred
Federal/state grant reporting, quality improvement, or outcomes tracking experience a significant plus
Rural or community health experience valued

Skills

Strong clinical documentation and EMR/EHR proficiency (Tebra a plus)
Detail-oriented data collection, organization, and reporting
Strong communication with rural facility staff, providers, and patients
Self-directed work style across multiple sites and competing deadlines
Working knowledge of HIPAA compliance

Certifications

Active clinical licensure (RN, LPN, LSW, CMA, or equivalent) preferred
HIPAA certification required (can be completed during onboarding)

Compensation & Benefits

$55,000–$65,000 salary
Health and dental insurance (75% of premiums covered)
