Pride Health is hiring an RN - Adult Care (Home Health/Medical-Surgical) to support our client’s medical facility based in New York, NY 10004. This is a 13-week assignment with the possibility of a contract-to-hire opportunity and a great way to start working with a top-tier healthcare organization!

Job Title: RN - Adult Care (Home Health/Medical-Surgical)

Location: New York, NY 10004

Pay Range (W2) Local: $70 - $76 per hour

Schedule: 7.5-hour day (Monday–Friday, 9:00 AM – 05:00 PM), 37.5 hrs/week

Start date: 06/15/2026

Role Overview:

We are seeking a compassionate and experienced Registered Nurse (RN) – Adult Care/Home Care to provide skilled nursing services to adult patients in their homes. This role involves comprehensive patient assessments, medication management, wound care, patient education, care coordination, and ongoing monitoring of patient health status. The ideal candidate will have recent home care and Med/Surg experience, strong clinical assessment skills, and proficiency with OASIS documentation.

Required Qualifications:

Assess, plan, implement, and evaluate individualized patient care plans in collaboration with the healthcare team.
Provide skilled nursing care to adult patients in home and community settings.
Perform comprehensive physical assessments and ongoing patient monitoring.
Administer medications, IV therapies, and prescribed treatments safely and accurately.
Perform wound care, dressing changes, catheter care, and infection prevention interventions.
Monitor vital signs, identify changes in patient condition, and communicate findings appropriately.
Educate patients and caregivers regarding disease management, medications, treatment plans, and safety measures.
Complete OASIS assessments and maintain accurate clinical documentation.
Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
Assist with discharge planning and continuity of care.
Maintain compliance with nursing standards, regulatory requirements, and agency policies.
Utilize EMR systems to document patient care and clinical outcomes.

Key Responsibilities:

Associate Degree in Nursing from an accredited nursing program required.
Active New York State Registered Nurse (RN) License in good standing required.
Current AHA BLS certification required.
AHA ACLS certification required where applicable.
Minimum two (2) years of recent Med/Surg experience in an acute care or post-acute setting.
Previous Home Care Nursing experience required.
Experience providing direct patient care in patients' homes required.
Knowledge and experience with OASIS documentation required.
Strong clinical assessment, medication administration, wound care, and IV therapy skills.
Experience with telemetry monitoring and post-operative patient care preferred.
EPIC EMR experience preferred.
Must be able to travel locally to patient homes and work every other weekend.
