Full Job Description

The Wound Care Nurse / Wound, Ostomy and Continence Nurse (WOCN) at Serenity Oaks IRF is the clinical expert responsible for the assessment, management, and treatment of patients with complex wounds, ostomies, and continence disorders throughout their inpatient rehabilitation stay. This specialist collaborates with the IDT, attending physicians, and nursing staff to develop individualized wound care plans, prevent pressure injuries, and promote skin integrity across the patient population. The WOCN serves as an educator and resource for nursing staff and contributes to QAPI and CARF program evaluation activities.

2. ESSENTIAL FUNCTIONS & RESPONSIBILITIES

Perform comprehensive wound, ostomy, and continence assessments on admission and at regular intervals for all assigned patients.

Develop and implement individualized wound care plans in collaboration with the attending physician and IDT.

Select and apply evidence-based wound care products, dressings, and treatment modalities.

Measure, stage, and document wounds using validated tools (PUSH Tool, Bates-Jensen Wound Assessment Tool, or facility-approved equivalent).

Implement and monitor pressure injury prevention protocols for all patients, including Braden Scale assessments.

Provide ostomy care management, pouching system selection, and peristomal skin management for patients with colostomies, ileostomies, or urostomies.

Assess and manage continence issues including urinary and fecal incontinence, catheter management, and bowel programs.

Consult with and educate nursing staff on wound assessment techniques, product selection, and care plan implementation.

Provide patient and family education on wound care, ostomy management, skin protection, and home care techniques.

Document wound status, treatment plans, and progress accurately and timely in the EHR.

Participate in IDT meetings to communicate wound care status and goals.

Coordinate with the attending physician for wound-related orders, consults, and physician notification of wound deterioration.

Contribute to QAPI activities; track wound care outcomes and drive improvement initiatives.

Maintain a current formulary of wound care products and advise purchasing on evidence-based product selection.

3. EDUCATION & EXPERIENCE REQUIREMENTS

Required: Bachelor of Science in Nursing (BSN) from an accredited nursing program.

Required: Minimum two (2) years of clinical nursing experience; wound care experience preferred.

Preferred: Completion of a WOCN Society-approved wound, ostomy, and continence nursing education program.

Preferred: IRF, acute care, or post-acute nursing experience with complex wound populations.

4. LICENSURE, CERTIFICATION & REGISTRATION REQUIREMENTS

Required: Current, unrestricted Registered Nurse (RN) license issued by the Louisiana State Board of Nursing (LSBN).

Required: Current BLS/CPR certification (AHA or Red Cross).

Preferred: Wound, Ostomy and Continence Nurse (WOCN) certification from the Wound, Ostomy and Continence Nursing Certification Board (WOCNCB).

Preferred: Certified Wound Care Nurse (CWCN), Certified Ostomy Care Nurse (COCN), or Certified Wound Specialist (CWS) credentials.

Required: Louisiana RN license verified through LSBN prior to and throughout employment.

5. KNOWLEDGE, SKILLS & ABILITIES

Expert knowledge of wound assessment, staging, and evidence-based wound care management.

Proficiency in ostomy care, pouching systems, and peristomal skin management.

Knowledge of continence management, urinary catheter care, and bowel program development.

Ability to perform and interpret Braden Scale assessments for pressure injury risk.

Knowledge of wound care product formularies and evidence-based product selection.

Effective communication skills for education of patients, families, and nursing staff.

Proficiency with EHR documentation of wound assessments and care plans.

Understanding of CMS and CARF requirements related to skin and wound care in IRF settings.

Ability to collaborate with IDT members on complex wound care cases.

Commitment to evidence-based, patient-centered wound care practice.

License/Certification:
