Full Job Description

Registered Nurse (RN) – LGBTQIA+ Health Clinic | Identity Hormones and Healthcare

Location: Phoenix, AZ
Schedule: Monday–Friday, as needed shifts | No nights, weekends, or on-call
Job Type: Per Diem, On-Site/In-Person
Compensation: $40 per hour

Identity Hormones and Healthcare is a specialty medical clinic that provides affirming, accessible, patient-centered care for the LGBTQIA+ community and its allies. With clinics across Phoenix, Denver and Salt Lake City — and growing quickly — we are seeking a compassionate and skilled Registered Nurse (RN), per diem to join our Phoenix team.

Our services include gender-affirming hormone therapy for transgender men and women, testosterone replacement therapy (TRT) for cisgender adults, HIV/PrEP management, STD/STI testing and treatment, weight management, and concierge-style wellness care. Our nurses play a key role in delivering exceptional, relationship-focused care every day.

What You’ll Do Day-to-Day

As an RN at Identity, you’ll provide hands-on clinical care in a comfortable, wellness-focused outpatient environment — no nights, no weekends, no call, and no sick patients. You’ll build meaningful connections with patients while supporting their health and transition journeys.

Daily responsibilities include:

Direct Patient Care

Performing patient assessments, reviewing medical histories, and documenting care in the EMR
Performing phlebotomy, IV therapy, and intramuscular or subcutaneous injections
Administering weekly hormone injections and monitoring patient responses
Supporting gender-affirming care protocols, including labs, dosing schedules, and ongoing monitoring
Assisting with PrEP/HIV care, including labs, education, and follow-up
Collecting specimens and performing in-clinic STD/STI testing

Education & Relationship Building

Educating patients on hormone therapy, wellness expectations, and treatment plans
Coaching patients on lifestyle, nutrition, and medication adherence
Providing compassionate, affirming support to transgender, nonbinary, and gender-diverse patients

Clinical Standards & Operations

Ensuring all treatments meet Identity’s clinical safety, quality, and compliance standards
Maintaining accurate EMR documentation
Contributing to a positive, inclusive, team-oriented clinic culture
Supporting clinic flow, efficiency, and exceptional patient experience

Detail-oriented and experienced with EMR documentation

Requirements

Active Registered Nurse (RN) license (required)
Minimum 1 year of nursing experience (preferred)
Phlebotomy, IV therapy, and injection experience
EMR proficiency and strong clinical documentation skills
Ability to work respectfully and effectively with diverse populations
