Reports To: Director of Nursing/Clinical Manager

Job Summary

Directs, supervises, coordinates and evaluates patient care and services provided by agency personnel. Responsible for overseeing licensed professionals as well as Home Health Aides and CNA's. Provide skilled care, if indicated.

Initial and ongoing comprehensive assessments of the patient’s needs, including Outcome and Assessment Information Set (OASIS) assessments at appropriate time points.
Development and evaluation of the plan of care in partnership with the patients, representative (if any), and caregivers. Revising the plan of care, as necessary, and monitoring the appropriateness of services for the patient.
Providing skilled nursing services that are ordered by the physician or allowed practitioner, under a patient’s plan of care, which can be safely provided in the home, within his/her scope of practice, and reflect upon and support the agency’s mission, vision, and values.
Providing those services and/or treatments requiring specialized nursing skill.
Provides services in accordance with applicable agency policy and procedure as well as state and federal law and regulation and industry standards. Supports and implements accepted professional standards of community nursing and home health care practice in the provision of these services.
Counseling the patient and family in meeting nursing and related needs.
Meeting all documentation requirements according to agency policy and procedure. Preparing clinical and progress notes.
Assistance with coordination of services. Participating in case conferences, team meetings, and staff meetings as indicated.
Assessing home health aides and CNA’s for competency as it relates to the performance of their job duties. Completing documentation regarding his/her assessment of the aide/CNA.
Referral to other services as needed. Communicating with the physician and other healthcare personnel to inform them of any changes in the patient’s condition, assessed needs, and regarding any deviation from the patient’s plan of care.
Assigning selected portions of patient care to licensed practical nurses and home health aides but always retaining the full responsibility for the care given.
Planning for the discharge from service.
Overseeing the clinical record for each patient to whom services are provided. Completing patient record reviews, conferences, and taking responsibility for ongoing communication.
Additional duties, as assigned.

QUALIFICATIONS, EXPERIENCE, EDUCATION, AND TRAINING REQUIREMENTS:

Must be a graduate of an approved school of professional nursing and be licensed as a Registered Nurse in the State of Florida.
