Full Job Description

IMPORTANT – PLEASE READ

Must have home health experience
Must be able to document independently in Kinnser (WellSky) from day one
This is a field-based PRN role (not outpatient or hospital-based)
Must be comfortable working independently in patients' homes

Pay

Routine treatment visit: $65–$75 per visit
PT Evaluation (non-OASIS): $95–$105 per visit
Recert (reassessment): $80–$90 per visit
Discharge: $75–$85 per visit
Mileage reimbursed at IRS rate

Final rate within range is based on experience, availability, and coverage area.

Job Type

PRN / Per Diem (W-2)

Primary coverage area: Tarrant County, Dallas County, Collin County, and Denton County. Assignments are based on your preferred coverage area and availability—we work with you to keep visits geographically reasonable.

What You'll Do

Perform PT evaluations, treatment visits, reassessments, and discharges in patients' homes
Develop and implement plan of care in coordination with RN and physician
Document accurately and on time in Kinnser (WellSky Home Health)
Communicate patient progress and needs with the care team
Complete and submit documentation within 24 hours of each visit

OASIS assessments are managed by the RN. PT responsibilities focus on clinical evaluation, treatment, and documentation.

Requirements

Active Texas PT license (in good standing)
Current BLS/CPR
Valid driver's license, reliable vehicle, and auto insurance
Working knowledge of Medicare home health timelines
Ability to manage schedule and documentation independently

Preferred

Prior PRN home health experience in DFW
Experience with post-acute orthopedic recovery, fall-risk patients, CHF/deconditioned patients, and neuro cases (Parkinson's, stroke, etc.)
