With a focus on both pediatric and geriatric populations, the company has established itself as a trusted leader in delivering compassionate care across multiple communities.

Overview

The Providers Coordinator – Clinician owns the Staffing function and represents Infinite to our providers. This is a full-time, clinician-based ownership role that combines patient treatment with leadership, operational management, and staff development. The position is filled by experienced treating clinicians (PT / OT / ST); their salaries sit within treating-provider calculations and are partly offset by reimbursements. The Providers Coordinator oversees daily provider operations, drives clinical excellence, and is supported by a dedicated Providers Admin.

Duties and Responsibilities

Provide Ownership, Coverage, and Recruitment

Oversee daily provider operations with full clinician-based ownership Ensure provider coverage for the required volume of patients and visits.
Ensure provider coverage for the required volume of patients and visits.
Responsible for provider coverage, including internal recruitment and hiring of new providers to meet staffing goals.
Drive provider growth and maximize provider schedules; optimize the full-time to per-diem ratio.
Support provider retention through consistent communication, feedback, and resolution of concerns

Scheduling, Compliance & Clinical Quality

Schedule and assign providers based on care plans, location, availability, and regulatory requirements; keep schedules current in Raintree / Skylight and fill open cases promptly
Train providers in our systems (Raintree / Legacy) and on company regulations and expectations
Monitor provider performance, utilization, attendance, and document timeliness to ensure quality and compliance.
Support providers in understanding expectations, workflows, and performance benchmarks
Ensure services are delivered per the plan of care and applicable federal/state regulations (CMS, DOH); maintain provider credentials and certifications.
Combine patient care with leadership, operational management, and staff development

Accountability

Accountable for reimbursement vs. cost — gross profit (GP1).
Accountable for high-quality patient care and clinical excellence.
Manage staff performance
Address provider-related issues and ensure optimal patient-care outcomes.
Analyze data to identify trends and process-improvement areas for providers

Required Skills & Qualifications

Graduate of an accredited PT, OT, or SLP program; current, unrestricted New York State license.
Clinical experience in home health or a related setting; leadership or supervisory experience preferred.
Knowledge of therapy services, Medicare/Medicaid regulations, and home health documentation and billing.
Proficiency in EHR / clinical documentation systems (e.g., Odoo, Raintree) and Microsoft Office.
Strong organizational, communication, and problem-solving skills; team-oriented with integrity and dependability.

Work Environment

Full-time, 40 hours.
Combining a treating caseload with provider-coordination leadership.

Pay: $110,000.00 - $120,000.00 per year
