Job Description
At Vitasora Health, we believe care should never stop at the clinic door. The Patient Enrollment Specialist is the first voice many of our patients hear, and that matters. This is a high-volume outbound phone role in a fast-paced, performance-driven environment. Your job is to connect eligible patients with life-changing Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs through confident outbound calling, clear communication, and genuine care. Core Responsibilities
Outbound Enrollment
- Make high-volume outbound calls daily to eligible patients to introduce CCM and RPM services and obtain verbal enrollment consent.
- Present program benefits clearly and confidently, adapting your approach to each patient and their individual circumstances.
- Handle objections with patience, persistence, and professionalism.
- Meet or exceed daily and monthly enrollment targets.
Patient Communication
- Explain program participation, expectations, and next steps with clarity and empathy, particularly when speaking with seniors managing chronic conditions.
- Build trust quickly on every call and represent Vitasora Health with professionalism and care.
- Always maintain strict HIPAA compliance and patient confidentiality.
Documentation
- Accurately log all call outcomes, enrollment statuses, and patient interactions in the care coordination system.
- Track individual enrollment metrics and contribute to daily and weekly reporting.
Team Contribution
- Participate in team huddles and stay aligned on enrollment goals and program priorities.
- Share patient feedback and call insights with your team to continuously improve outreach effectiveness.
This role may require completion of tasks and responsibilities outside of those listed above, as business needs evolve. Flexibility and a willingness to contribute beyond defined expectations is an important part of this role.
Required Qualifications
- Minimum 1 year of experience in outbound sales for enrollment.
- General outbound call center, telemarketing, or a high-volume phone-based role.
- Demonstrated ability to meet daily quotas or performance targets in a metrics-driven environment.
- High school diploma or GED required.
- Strong verbal communication skills with a confident, empathetic, and professional phone presence.
- Comfortable with technology including VoIP systems, CRM or documentation platforms, and basic computer navigation.
- Reliable high-speed internet (minimum 50 Mbps) and a quiet, distraction-free home workspace.
Preferred Qualifications
- Experience in healthcare, insurance, or patient-facing enrollment is a plus but not required.
- Familiarity with CCM, RPM, Medicare, or telehealth services.
- Experience working with patient populations or patients managing chronic conditions.
- Proven ability to build rapport quickly and handle objections with empathy and persistence.
- Bilingual in English and Spanish is strongly preferred.

Schedule & Availability
This role requires flexibility in scheduling based on business needs, which will be communicated well in advance. Team members may be asked to adjust working hours, including early mornings, evenings, or occasional weekends, to support patient outreach goals, program deadlines, or operational demands. A willingness to work flexible hours is an important part of this role.
