About the Role
JACI Family Solutions is hiring an experienced Pregnancy Care Coordinator / Case Manager to support pregnant women and families in Milwaukee County.
This role is ideal for someone with real experience in human services, healthcare, social services, case management, care coordination, WIC, home visiting, public health, community outreach, family services, child welfare, or maternal and child health.
The Care Coordinator works under the supervision of the Qualified Professional and helps clients access medical, social, behavioral health, housing, food, transportation, parenting, and community resources. This position also requires strong follow-up, accurate case notes, timely documentation, and professional communication with the care team.
Important Note
This is not the Qualified Professional or Program Manager role.
This is also not an entry-level position. We are looking for someone who already has experience working with clients, patients, families, or community members in a human services, healthcare, social services, case management, care coordination, or family support setting.
Strong documentation skills are required.
Required Qualifications
Candidates must meet the following requirements:
- High school diploma or GED required
- Must be at least 18 years old
- Prior experience in human services, healthcare, family services, social services, case management, care coordination, community outreach, or a related field required
- Strong case note writing and documentation skills
- Ability to work under the supervision of the Qualified Professional
- Ability to complete required training before serving clients
- Ability to maintain confidentiality and follow documentation standards
- Reliable transportation or the ability to reliably serve clients in Milwaukee County
- Strong communication, organization, and follow-through skills
- Professional, dependable, compassionate, and able to work with pregnant women and families facing barriers
Preferred Qualifications
Preferred candidates may have:
- Associate’s or bachelor’s degree in human services, social work, public health, psychology, sociology, family services, health education, or a related field
- Experience working with pregnant women, moms, infants, families, low-income families, or high-risk communities
- Experience with Medicaid, ForwardHealth, WIC, home visiting, public health, community-based services, or family support programs
- Experience completing case notes, referrals, follow-up notes, assessments, care plans, or client documentation
- Knowledge of Milwaukee County community resources
- Experience supporting families with needs related to housing, food, transportation, prenatal care, behavioral health, parenting, safety, or social support
Key Responsibilities
The Pregnancy Care Coordinator will:
- Support pregnant women and families throughout pregnancy and postpartum
- Maintain regular client contact based on client needs and care plan goals
- Help clients access medical care, prenatal care, behavioral health services, housing, food, transportation, parenting support, and community resources
- Provide information and referrals to appropriate service providers
- Follow up on referrals and document the outcome
- Complete timely, accurate, professional case notes
- Document client contacts, referral activity, follow-up, barriers, progress, and next steps
- Support care plan goals under the supervision of the Qualified Professional
- Communicate client progress, needs, concerns, and barriers to the Qualified Professional
- Maintain confidential client records
- Participate in required training, supervision, case review, and team communication
- Provide compassionate, culturally appropriate, and professional support to families
- Work with community partners and support systems when appropriate and with proper client consent

Schedule
Flexible schedule based on client needs, supervision, team communication, and documentation deadlines.
This role may include phone contacts, virtual communication, community-based support, team meetings, documentation time, and in-person visits when needed.
