Job description

BrightStar Care of San Francisco is hiring an experienced Pediatric Clinical Case Manager to lead care coordination for our pediatric home care patients — including medically fragile children transitioning from hospital or NICU to home. If you're a clinically strong and compassionate professional who thrives on care coordination and family support, we'd love to talk.

We are a fast growing Pediatric Home Health agency care, looking for someone who wants to grow along with us.

BrightStar Care of San Francisco is a Joint Commission accredited company who has built a reputation for providing the highest level of care for our clients throughout the Bay Area. We are looking to add a dynamic leader to take the concierge business to the next level.

The ideal candidate will have a strong pediatric care case management background. Ability to solve problems independently, influence clients, their families and healthcare workers to develop care plans that ensure continuous high quality care.

Job Responsibilities

Be the go-to problem-solver for families caring for medically fragile babies at home who aren't sure what to do next
Troubleshoot day-to-day care issues alongside the nursing team — helping families and caregivers work through real-world challenges in the home
Help families access and resolve issues such as with DME (durable medical equipment) — ventilators, feeding pumps, tracheostomy supplies, and more
Connect families with other related services and resources their baby needs
Coordinate supervisory visits and reassessments per Title 22 timelines
Coordinate closely across caregivers, nurses, physicians, and families to keep everyone aligned
Serve as the clinical point of contact for families and field staff when questions or concerns come up
Maintain compliant documentation (Medi-Cal/Medicare standards)
Support and coach caregiving staff working on pediatric cases

Requirements

2+ years clinical experience (pediatric, home health, or NICU strongly preferred)
Case management or discharge planning experience a plus
Strong communicator, self-directed, comfortable coordinating across a care team
Spanish a plus
Ability to work well with both clients and employees
Proficient in MSOffice (Outlook, Word, Excel, and PowerPoint)
Flexibility and good time management skills
Demonstrated organizational skills
Excellent organization, planning, and project management skills.
