Pineland BHDD is seeking a dedicated Case Manager to provide intensive community-based support for individuals participating in the Assisted Outpatient Treatment (AOT) Program. This position focuses on helping individuals navigate behavioral health services, access community resources, strengthen support systems, and achieve meaningful recovery goals while reducing hospitalizations, homelessness, and criminal justice involvement
The AOT Case Manager works closely with participants, families, behavioral health providers, courts, and community partners to provide care coordination, advocacy, recovery support, and resource linkage. Using a person-centered, strengths-based approach, this role helps individuals overcome barriers to recovery and successfully live and thrive in the community.
What You'll Do
Case Management & Care Coordination
- Assist participants in developing and achieving Individual Recovery Plan goals
- Connect individuals with housing, treatment services, employment resources, and community supports
- Monitor service effectiveness and help adjust supports when needs change
- Coordinate care among providers, families, courts, and community agencies
Advocacy & Family Support
- Advocate for participants and families to ensure access to appropriate services
- Help individuals understand their rights, responsibilities, and available resources
- Encourage family participation in treatment planning and recovery efforts
Recovery Support & Community Engagement
- Utilize a strengths-based, person-centered approach to support recovery
- Identify strengths and barriers impacting recovery and community integration
- Assist participants with activities of daily living (ADLs) as needed
- Model recovery-oriented, trauma-informed, and anti-stigma practices
Team Collaboration
- Work closely with the AOT Clinical Team Lead and Peer Mentor
- Participate in interdisciplinary team meetings
- Assist with support groups and educational activities as assigned
Documentation & Professional Responsibilities
- Maintain participant confidentiality and HIPAA compliance
- Complete documentation and reporting requirements accurately and timely
Required Qualifications
- Bachelor's Degree
- Minimum of 3 years of experience in the behavioral health field
- Valid Georgia Driver's License
- Reliable transportation and current automobile insurance
- Ability to pass criminal background and drug screening requirements
- Strong written, verbal, and organizational skills
- Proficiency with Microsoft Office, smartphones, and electronic systems
Preferred Qualifications
- Certified Peer Specialist (CPS-MH, CPS-AD, or CPS-P), or ability to obtain certification within one year
- Masters Degree Preferred
- Knowledge of Georgia's behavioral health and criminal justice systems
- Experience with community-based recovery support services
