Full Job Description

Life With Dignity is looking for a part-time/possible full-time, Case Manager within our growing business based in our St. Charles, IL headquarters. Our team of Case Managers work in office and client field environments supporting our clients throughout the Chicago area. Life With Dignity delivers advocacy and support for individuals dealing with disabilities or challenges due to illness or incapacity. We assist clients through health, psychosocial, financial, and residential needs taking pride in going the extra mile for them and their families.

Our Case Management services include coordination of Advocacy, Life/Care Planning, facilitating Case Management, serving as Agent under the power of attorney, Executor, Trustee and Guardianship services.

The Case Manager reports to the Director of Case Management (DCM) and delivers effective, client-focused, and profitable results in collaboration with our Case Management, Property Management, and Estate Management teams.

The ideal candidate brings extensive experience serving the elder and disabled communities delivering effective coordination within interdisciplinary environments including healthcare, legal, and asset management systems. Your high sense of duty and integrity will align with Life with Dignity’s commitment to excellence in communications with others - both externally and internally.

You will bring extensive knowledge-based experience with Medicaid / Medicare cases and long-term care policies. Knowledge of Mental health services and developmental disabilities services would be of great benefit to our team and our clients.

Success in this role is achieved through the effective oversight and coordination of assigned client cases while serving in the designated support role in alignment with LWD’s core values. The Case Manager is responsible for overseeing and coordinating all aspects of client needs, care planning, transitions, and service execution to support client stability, safety, dignity, continuity of care, and goal attainment. Day-to-day responsibilities include maintaining consistent progress on active cases; coordinating client services, appointments, resources, and transitions of care; executing tasks accurately, professionally, and in a timely manner; ensuring thorough, accurate, and compliant documentation within designated systems; proactively identifying and addressing client needs, barriers, and challenges; and maintaining ongoing communication and collaboration with the Director of Case Management, internal departments, care providers, fiduciaries, and the Estate and Property Management team to support continuity of care and optimal client outcomes.

The Case Manager responsibilities include:

Monitor and review Cases to facilitate comprehensive individualized care / service plans performing client best solutions for healthcare, housing, and financial oversight considerations while documenting progress toward established care plan goals.
Initiate and follow up timely on communications between the Estate and Property Management team to ensure effective client focused results that leverage both client needs and financial resources.
Escalate care coordination challenges to the Director of Case Management for guidance and support in resolving barriers to effective, timely care and planning.
Ensure proper procedures and documentation in all efforts to stay compliant, protective of client needs, and profitable
Maintain current professional working knowledge of guardianship laws and processes, including roles and responsibilities of court-appointed guardians as pertains to Life With Dignity clients.
Facilitate successful coordination with guardians, conservators, attorneys, and court representatives to ensure compliance with care plans and legal directives.
Daily maintain timely, accurate and complete documentation and billable notes staying consistent with court reporting and regulatory requirements.
Advocate with DCM and other Case Managers for the least restrictive alternatives while promoting client autonomy and safety through appropriate and cost-effective services.
Maintain collaborative relationships for services across healthcare providers, community agencies, and internal departments.
Request and facilitate completion of proper referrals for medical care, behavioral health services, housing support, financial assistance, or protective services for clients.
Concurrent documentation of all interdisciplinary coordination and case outcomes.
Participate as a part of LWD team to meet or exceed company goals while embodying professionalism and LWD’s core values in all efforts and interactions.
Attend all assigned meetings, prepared and timely.

Required for Success:

Project / Case Management experience - disability/elderly community - Min 4 yrs
Experience managing cases of 10 or more at the same time
Experience advocating on appointments, court appearances, and other off-site environments
Professional Knowledge-based experience with guardianship/conservatorship, Min 1 yr
Professional knowledge-based experience of Medicare/Medicaid benefits
High level of Integrity / honesty
Compassionate
Excellent communication – verbal and written
Logistics Excellence in time management and follow up processes to serve offsite appointments and client needs in the field.
Budgetary constraints and authorization approval processes
Excellent experience with online solutions including but not limited to email, government and agency systems, project management, etc.
Strong organization and follow-up skills
Professional experience using a De-Escalation strategy

Prefer, though not required:

Medicare/Medicaid certification
Guardianship Certification

Compensation:

The part-time positions compensated $26-$38 per hour, based upon experience.

Benefits:

Combination remote and office position.

This role provides variability in scheduling, with client needs and case priorities serving as the primary driver of all work hours.

Background checks: All successful candidates will be required to pass a caregiver/criminal/financial background check, clean driving record, and provide proof of vehicle insurance.
