Wellness Care provides Chronic Care Management (CCM) and care coordination services for Medicare beneficiaries. We’re building a highly compliant, patient-first operation—powered by strong clinical teams and clean, audit-ready documentation—so our patients get consistent support, and our billing is done the right way.

The Opportunity

We’re hiring a Medical Coder & Biller to support our Medicare billing workflow—primarily CCM and E/M visit coding and are supported by documentation and program design. You’ll partner closely with clinical and operations teams to ensure accurate coding, compliant billing prep, fewer denials, and audit-ready records.

What You’ll DoCoding & Chart Review

· Review clinical documentation to ensure compliant coding and billing (time capture, consent, care plan elements, monthly service requirements, and E/M documentation standards).

· Assign accurate ICD-10-CM codes aligned to chronic conditions and documented needs, including SDOH Z-codes when supported (e.g., Z55–Z65 and other relevant Z-codes tied to documented social risk).

· Select correct CCM CPT/HCPCS codes (as applicable), including: 99490, 99439, 99487, 99489, 99491, 99437.

· Code and validate E/M visits (as applicable to Wellness Care workflows), including:

o Office/Outpatient E/M: 99202–99205, 99212–99215

o Add-on when applicable/allowed: G2211

· Code RPM services (as applicable) and ensure documentation supports setup/education, device supply, and time-based monitoring/management, including: 99453, 99454, 99457, 99458, 99091.

· Code SDOH assessment/interventions when supported by documentation and payer policy (as applicable), including: G0136.

· Additional codes added as programs expand

· Identify documentation gaps and coordinate with clinical teams to resolve issues prior to billing.

Billing Prep & Claims Support

· Prepare claim-ready data and billing packets (code/diagnosis validation, supporting documentation indicators).

· Verify prerequisites are met (e.g., required elements completed per internal workflow).

· Support submission of readiness, corrections, resubmissions, and documentation requests.

· Work denials/rejections: identify root causes, correct issues, and prevent recurrence.

Compliance & Audit Readiness

· Maintain audit-ready coding rationale and billing support documentation for Medicare services.

· Perform routine internal quality checks and provide findings + corrective actions.

· Escalate compliance risks quickly (e.g., missing consent/time support, insufficient E/M support, incomplete care plan elements, overlapping services).

Team Collaboration & Process Improvement

· Partner with Care Managers (RNs), Care Professionals, enrollment/ops, and supervising providers to strengthen documentation quality.

· Create job aids and quick trainings for documentation behaviors that support compliant billing (CCM time capture, E/M documentation standards, SDOH documentation best practices, RPM documentation requirements).

· Improve templates, checklists, and workflows to increase clean claims and reduce rework.

What We’re Looking For: Qualifications

Required:

· 2+ years of medical coding and/or billing experience with Medicare exposure

· 2+ years of experience working as a Certified Medical Coder with billing experience and knowledge

· Certifications: CPC, CCS, CRC, CPB (AAPC/AHIMA)

· Strong working knowledge of ICD-10-CM and CPT/HCPCS

· Experience coding E/M services and applying documentation requirements

· Familiarity with CCM billing/documentation expectations (time-based services, consent, care plan requirements, monthly billing rules)

· Comfort coding SDOH when supported by documentation and payer requirements

· Comfort coding RPM when supported by documentation and program requirements

· Experience working in EHR/EMR workflows and coordinating with clinical teams

· High attention to detail, comfort with audits, and ability to work independently

· Ability to resolve discrepancies for coding-related issues· Prepare and submit billing data and medical claims through the company’s EMR system Experience coding E/M services and applying documentation requirements

Preferred:

· Hands-on experience with CCM/RPM billing or care management programs

· Denials management experience and ability to translate trends into workflow fixes

· Experience in telehealth, care management, or high-volume outpatient billing environments

Pay: $30.00 - $35.00 per hour

Expected hours: 40 per week
