Position: Intake Specialist
Reports To: Director of Outreach
Position Summary:
The Intake Specialist plays a critical role in linking community members to IDCC’s network of behavioral health, medical, and supportive services. This position combines outreach, engagement, education, and intake functions to ensure individuals and families gain access to the care and resources they need. The Intake Specialist will conduct outreach and intake activities for IDCC’s Health Home Program and other IDCC service lines. This includes completing screenings, eligibility assessments, and enrollments while engaging community partners, representing IDCC at outreach events, and promoting our full continuum of care. This position requires a strong understanding of human services systems, care coordination principles, and the ability to communicate effectively with diverse populations while maintaining the highest standards of confidentiality and professionalism. This is an in-person position, Monday through Friday, requiring approximately 75% travel throughout the five boroughs of New York City to conduct outreach, community engagement, and in-home client assessments.
Essential Duties and Responsibilities
Client Engagement & Enrollment
- Provide comprehensive education to potential clients, explaining the benefits of care coordination and IDCC services to encourage participation.
- Conduct initial screenings, eligibility assessments, and consents as part of the intake process for the Health Home Program and all other IDCC related services.
- Assessments and intakes may be completed in-home, in-person at community or clinic sites, or telephonically as needed to accommodate client accessibility and program requirements.
- Utilize approved databases, internet resources, and electronic health record systems to verify Medicaid eligibility and collect client contact information.
- Track and document all outreach attempts, enrollments, and follow-up activities in accordance with agency and regulatory requirements.
- Maintain accurate and confidential client records using designated software platforms (e.g., CareLogic, Foothold, UniteUs, etc.).
- Meet monthly enrollment goals of 20–25 successful Health Home enrollments and maintain a success rate of 25–30%.
Community Outreach & Engagement
- Conduct community outreach and tabling events to promote IDCC’s programs and build relationships with partner organizations, schools, housing programs, and other providers.
- Engage community members and providers by educating them on the benefits of IDCC’s comprehensive care coordination and clinic services.
- Represent IDCC at community events, fairs, and meetings to increase visibility and promote program awareness.
- Establish and maintain strong partnerships with community-based organizations to support coordinated care and referrals.
- Participate in the planning and implementation of marketing and outreach strategies to support organizational goals.
Administrative & Compliance
- Perform diligent search efforts for potential client assignments under the Health Home Medicaid Program, including outreach through phone calls, letters, and community or home visits.
- Regular fieldwork is required, including travel between site locations and community settings using mass transit.
- Track outreach efforts in compliance with NY State Medicaid, Lead Health Home, Department of Health (DOH), and Office of Mental Health and Hygiene (OMH) regulations.
- Maintain and track caseload assignments and outcomes.
- Complete weekly reporting forms and other administrative documentation accurately and on time.
- Perform general clerical tasks such as filing, assembling/disassembling client charts, running Medicaid checks, developing client contact sheets, and processing mail or follow-ups.
- Maintain client confidentiality in accordance with HIPAA and organizational policies.
Organizational Participation
- Participate in team meetings, supervision, and professional development activities.
- Actively engage in agency events, initiatives, and collaborative projects to support IDCC’s mission.
- Contribute feedback and innovative ideas for improving outreach and engagement practices.
Qualifications:
- Education: Bachelor’s degree in Human Services, Social Work, Psychology, Public Health, or a related field (required).
- Minimum of one (1) year of experience in Human Services, Case Management, Outreach, or Community Engagement (required).
- Experience using Google Suite and Microsoft Office programs (required).
- Proficiency with various productivity tools and a willingness to learn new systems as needed.
- Experience with electronic databases or EHR systems preferred.
- Ability to operate with purpose, urgency, and accuracy in a fast-paced, deadline-driven environment while maintaining strict client confidentiality.
- Ability to work independently in the field and collaboratively as part of a multidisciplinary team.
- Exceptional communication, organization, and customer service skills.
- Strong understanding of care coordination and community-based service delivery.
- Knowledge of community resources and diverse client populations is required.
- Experience in sales or customer service is a plus, given the performance-based metrics associated with outreach and enrollments.
- Must be comfortable conducting in-home visits and community-based work throughout the five boroughs, often utilizing public transportation.
- Bilingual proficiency (required), in Spanish, Russian, Haitian Creole, French, Cantonese, Mandarin, Yiddish, or Hebrew is a strong plus.
- Live within the 5 boroughs (required).
